Fingolimod (Gilenya) Protocol for Relapsing-Remitting Multiple Sclerosis
Fingolimod 0.5 mg once daily is the standard dose for adults and pediatric patients ≥10 years weighing >40 kg; use 0.25 mg once daily for pediatric patients ≥10 years weighing ≤40 kg. 1
Pre-Initiation Requirements
Before starting fingolimod, complete these mandatory assessments 1:
Cardiac Evaluation
- Obtain ECG in all patients before first dose
- Evaluate for cardiac contraindications: recent MI, unstable angina, stroke/TIA, decompensated heart failure, Class III/IV heart failure, Mobitz Type II or 3rd degree AV block (unless pacemaker present), baseline QTc ≥500 msec, or arrhythmias requiring Class Ia/III antiarrhythmics 1
- Review all medications that could slow heart rate or AV conduction 1
Laboratory Testing
- Complete blood count (CBC) - review recent results 1
- Liver function tests (ALT, AST, total bilirubin) - must be within 6 months of initiation 1
Ophthalmologic Assessment
- Baseline fundus examination including macula evaluation near treatment start 1
- Repeat at 3-4 months after initiation, then periodically during therapy 1
- Diabetes and uveitis increase macular edema risk 1
Infectious Disease Screening
- Test for varicella zoster virus (VZV) antibodies - vaccinate antibody-negative patients before starting fingolimod 1
- Complete all pediatric immunizations per current guidelines before initiation if possible 1
Skin Examination
- Baseline skin exam prior to or shortly after initiation 1
First-Dose Monitoring Protocol
All patients require 6-hour observation after the first dose due to transient bradycardia and AV conduction effects 1.
Standard 6-Hour Monitoring 1
- Administer first dose in setting with resources to manage symptomatic bradycardia
- Hourly pulse and blood pressure measurements for 6 hours
- ECG before dosing and at 6 hours post-dose
Extended Monitoring Required If:
Continue monitoring beyond 6 hours until resolution if any of the following occur 1:
- Heart rate <45 bpm (adults), <55 bpm (age ≥12 years), or <60 bpm (age 10-11 years) at 6 hours
- Heart rate at 6 hours is the lowest postdose value (suggesting maximum effect not yet reached)
- New-onset 2nd degree or higher AV block on 6-hour ECG
- Symptomatic bradycardia - initiate continuous ECG monitoring until symptoms resolve
Overnight Monitoring Indications 1
Continuous overnight ECG monitoring required for:
- Patients requiring pharmacologic intervention for symptomatic bradycardia (repeat first-dose monitoring with second dose)
- Preexisting heart/cerebrovascular conditions
- Prolonged QTc before or during observation, or concurrent QT-prolonging drugs with torsades risk 2
- Concurrent therapy with heart rate or AV conduction-slowing drugs
Reinitiation After Discontinuation
Repeat full first-dose monitoring if fingolimod discontinued for >14 days after the first month of treatment 1. This also applies to dose increases in pediatric patients 1.
Ongoing Monitoring During Treatment
Routine Assessments
- Liver enzymes periodically during treatment - discontinue if liver injury occurs without other cause 1
- Ophthalmologic evaluation at 3-4 months, periodically thereafter, and with any vision changes 1
- Monitor for infections during treatment and for 2 months after discontinuation 1
MRI Surveillance
While the provided evidence focuses on natalizumab monitoring 3, patients switching from natalizumab to fingolimod require enhanced pharmacovigilance with brain MRI every 3-4 months for up to 12 months to detect opportunistic infections including PML 3.
Special Populations
Pediatric Patients
- Age ≥10 years, weight >40 kg: 0.5 mg daily 1
- Age ≥10 years, weight ≤40 kg: 0.25 mg daily 1
- Different heart rate thresholds for extended monitoring (see above) 1
Patients on Concomitant Medications
- SSRIs: No increased cardiac risk with fingolimod initiation; standard monitoring applies 4
- Disease-modifying therapies: Consider additive immunosuppressive effects before initiating 1
COVID-19 Vaccination Timing
Patients on fingolimod (sphingosine-1-phosphate receptor modulator) can be vaccinated at any time during treatment, though vaccine response may be attenuated 2.
Common Pitfalls to Avoid
- Failing to repeat first-dose monitoring after >14 day treatment gap - bradycardia effects recur 1
- Inadequate monitoring of high-risk cardiac patients - overnight telemetry prevents serious complications 1
- Missing baseline ophthalmologic exam - macular edema occurs in 0.3-1% and requires early detection 1
- Starting during active infection - fingolimod increases infection risk; defer initiation 1
- Inadequate contraception counseling - fingolimod causes fetal harm; requires effective contraception during and 2 months after treatment 1
