Warfarin Initiation with 2.5 mg Tablets
Start with 2.5 mg daily (one tablet) for most patients, or use two tablets (5 mg) for younger, otherwise healthy patients without bleeding risk factors.
Recommended Initiation Strategy
The FDA-approved warfarin label explicitly states that therapy should be initiated with 2 to 5 mg per day, with lower doses considered for elderly/debilitated patients and those with genetic variations or higher bleeding risk 1. Large loading doses increase hemorrhagic complications without providing faster protection against thrombosis and are not recommended 1.
Dosing Algorithm Based on Patient Characteristics:
Use 2.5 mg daily (1 tablet) for:
- Elderly patients (≥75 years) - they require approximately 1 mg/day less than younger individuals 2
- Debilitated patients
- Patients with bleeding risk factors (history of GI bleeding, stroke, renal insufficiency, anemia) 2
- Patients on concomitant antiplatelet agents or NSAIDs 2
Use 5 mg daily (2 tablets) for:
- Younger, otherwise healthy patients without the above risk factors
- Patients requiring therapeutic anticoagulation urgently (though still avoid true "loading doses")
Monitoring and Adjustment Protocol
- Check INR daily after the initial dose until results stabilize in therapeutic range 1
- Most patients ultimately require maintenance doses of 2-10 mg daily 1
- The 2.5 mg tablets can be broken in half for 1.25 mg increments if needed 1
Important Clinical Pearls:
The 2.5 mg starting approach is particularly advantageous because research shows that in post-cardiac surgery patients, a 2.5 mg fixed dose resulted in significantly fewer excessive INRs (26.2% vs 42.5% with 5 mg, p<0.05) and fewer INRs >3.0 on day 3 (9.5% vs 23.9%, p<0.05) 3. While time to therapeutic range was slightly longer (2.72 vs 1.98 days), the approach was more regular with fewer dose adjustments needed 3.
Avoid the common pitfall of using 10 mg loading doses - evidence shows no clear benefit over 5 mg for achieving stable therapeutic anticoagulation, and higher initial doses increase bleeding risk 4, 5.
Special Considerations
For very elderly patients (>75 years) or those with multiple bleeding risk factors, consider starting even lower at 1.25 mg (half a tablet) daily, as these patients demonstrate increased sensitivity to warfarin 2, 6.
The anticoagulant effect persists beyond 24 hours, so daily dosing is appropriate regardless of tablet strength available 1.
Bridge therapy with heparin or LMWH should be used concurrently until INR reaches therapeutic range for at least 24 hours in patients requiring immediate anticoagulation (acute DVT, PE, high-risk AF) 1.
