Vibegron Indication
Vibegron (GEMTESA) is FDA-approved for treating overactive bladder (OAB) with symptoms of urge urinary incontinence, urgency, and urinary frequency in adults, including adult males concurrently taking pharmacological therapy for benign prostatic hyperplasia (BPH). 1
Primary Indications
Vibegron is indicated for two specific adult populations:
General adult population with OAB presenting with:
- Urge urinary incontinence (UUI)
- Urgency
- Urinary frequency 1
Adult males with OAB on BPH pharmacotherapy (specifically α-blockers ± 5α-reductase inhibitors) experiencing the same triad of symptoms 1
Mechanism and Clinical Context
Vibegron functions as a selective beta-3 adrenergic receptor agonist that increases bladder capacity by relaxing detrusor smooth muscle during bladder filling 1. This represents a distinct mechanism from antimuscarinic agents, offering an alternative for patients who cannot tolerate or fail anticholinergic therapy.
Key Clinical Evidence
The EMPOWUR trial (phase III, n=1,518) demonstrated that 75 mg once-daily vibegron achieved:
- 1.8 episodes/day reduction in micturitions vs 1.3 for placebo (p<0.001)
- 2.0 episodes/day reduction in UUI vs 1.4 for placebo (p<0.0001) 2
The COURAGE trial specifically validated safety and efficacy in men with OAB receiving BPH treatment, showing minimal impact on postvoid residual volume and urinary flow rates, with urinary retention rates comparable to placebo (0.9% vs 0.7%) 3.
Dosing and Administration
- Standard dose: 75 mg orally once daily 1
- Can be taken with or without food
- May be swallowed whole or crushed and mixed with applesauce 1
Important Contraindications and Limitations
Do not use in:
- Prior hypersensitivity to vibegron or product components 1
- End-stage renal disease with or without hemodialysis (not recommended) 1
- Severe hepatic impairment (Child-Pugh C) - not recommended 1
Critical Safety Considerations
Monitor for urinary retention, particularly in:
- Patients with bladder outlet obstruction
- Patients taking concomitant muscarinic antagonists (additive risk) 1
Angioedema risk: Facial and/or laryngeal angioedema has been reported; discontinue immediately if this occurs 1.
Digoxin interaction: Vibegron increases digoxin levels (Cmax by 21%, AUC by 11%); measure baseline digoxin concentrations before initiating and monitor to titrate appropriately 1.
Comparative Advantages
Real-world data demonstrates vibegron's superior adherence profile compared to both mirabegron and anticholinergics, with longer persistence (median 171-172 days vs 128 days for mirabegron and 91 days for anticholinergics) 4. This likely reflects vibegron's high beta-3 receptor selectivity and lack of cytochrome P450 interactions, making it particularly advantageous in older patients with polypharmacy 5, 6.
