While some patients may respond to lower doses (100-150 mg daily), research shows that many OCD patients require the full 200 mg dose for optimal symptom reduction 1.
It's essential to consider the optimal duration of treatment, as OCD treatment guidelines indicate that 8–12 weeks is the optimal duration of an SSRI trial to determine efficacy 1. The recommended maintenance duration of pharmacotherapy is a minimum of 12–24 months after achieving remission, but longer treatment might be necessary in many patients owing to the risk of relapse after discontinuing medication 1.

The effectiveness of sertraline in the treatment of OCD was demonstrated in three multicenter placebo-controlled studies of adult outpatients (Studies 1-3) Patients in all studies had moderate to severe OCD (DSM-III or DSM-III-R) with mean baseline ratings on the Yale–Brown Obsessive-Compulsive Scale (YBOCS) total score ranging from 23 to 25. Study 1 was an 8-week study with flexible dosing of sertraline in a range of 50 to 200 mg/day; the mean dose for completers was 186 mg/day Study 2 was a 12-week fixed-dose study, including sertraline doses of 50,100, and 200 mg/day Study 3 was a 12-week study with flexible dosing of sertraline in a range of 50 to 200 mg/day; the mean dose for completers was 185 mg/day
