Focalin Starting Dose
For pediatric patients (6 years and older) new to methylphenidate, start Focalin XR at 5 mg once daily in the morning; for adults, start at 10 mg once daily in the morning. 1
Dosing by Patient Population
Patients New to Methylphenidate
The FDA-approved starting doses are clearly defined:
- Pediatric patients (≥6 years): 5 mg once daily
- Adults: 10 mg once daily
Both doses should be administered in the morning with or without food 1.
Patients Currently on Methylphenidate
If converting from racemic methylphenidate, use half (1/2) the total daily dose of methylphenidate as the starting dose of Focalin XR 1. For example, a patient taking 20 mg/day of methylphenidate would start Focalin XR at 10 mg/day.
For patients already on immediate-release Focalin (dexmethylphenidate), give the same total daily dose as Focalin XR 1.
Titration Schedule
Titrate weekly in increments of:
- 5 mg for pediatric patients
- 10 mg for adult patients
Continue titration until symptoms are adequately controlled or side effects emerge 1.
Maximum Recommended Doses
- Pediatric patients: 30 mg/day
- Adults: 40 mg/day
Daily doses above these limits have not been studied and are not recommended 1.
Important Clinical Context
The guideline evidence, while from 2002, provides context that dexmethylphenidate (DEX) starting doses of 2.5 mg were recommended for immediate-release formulations 2. However, the FDA label for Focalin XR supersedes these older guidelines and specifies 5 mg for pediatric patients and 10 mg for adults for the extended-release formulation 1.
Critical Caveat for Young Children
Focalin XR is not recommended for children younger than 6 years due to higher plasma exposure and increased incidence of adverse reactions (particularly weight loss) compared to older children at the same dosage 1.
Administration Flexibility
Capsules may be swallowed whole or opened and sprinkled on applesauce for patients unable to swallow capsules. The entire contents must be consumed immediately without chewing, and the dose should not be divided 1.
Monitoring Requirements
Before initiating treatment, assess for cardiac disease (personal and family history of sudden death or ventricular arrhythmia) and evaluate for tics or Tourette's syndrome 1. Monitor blood pressure, pulse, height, and weight at baseline and throughout treatment.
