Lansoprazole (Junior Lanzol) Dosing for Children
For children aged ≥1 year, lansoprazole should be dosed at 0.7-3 mg/kg/day based on weight: 15 mg daily for children ≤30 kg and 30 mg daily for children >30 kg. 1, 2
Weight-Based Dosing Algorithm
Children 1-11 Years of Age
- ≤30 kg body weight: 15 mg once daily
- >30 kg body weight: 30 mg once daily
- Duration: 8-12 weeks for initial treatment 2
Adolescents 12-17 Years of Age
- Non-erosive GERD: 15 mg once daily
- Erosive esophagitis: 30 mg once daily
- Duration: 8-12 weeks 2
Dose Escalation Strategy
If symptoms persist after 2 or more weeks of initial treatment, the dose may be increased up to 30 mg twice daily (maximum 60 mg/day) 2, 3. In clinical trials, 24 of 66 children (36%) required dose escalation, with only one child experiencing a new treatment-related adverse event after increasing to 1.3 mg/kg/day 4.
Available Formulations
The medication can be administered as 1:
- Capsule contents sprinkled onto soft foods or select juices
- Through nasogastric tube (≥8 French) when mixed with juice
- Strawberry-flavored orally disintegrating tablet
- Via oral syringe
Clinical Efficacy Evidence
The dosing range of 0.7-3.0 mg/kg daily has demonstrated 100% healing of erosive esophagitis by 12 weeks and 76% improvement in overall GERD symptoms 1, 3. In the pivotal study of 66 children aged 1-11 years, 78% healed by week 8 and all remaining cases healed by week 12 3.
Critical Safety Considerations
Do NOT Use in Infants <1 Year
Lansoprazole is NOT indicated for infants under 1 year of age 2. Placebo-controlled trials showed no superiority over placebo for reducing irritability in this age group, with 54% response rates in both lansoprazole and placebo groups 2.
Important Caveats
- Do not exceed recommended dose and duration in pediatric patients due to potential bone changes and heart valve thickening observed in juvenile animal studies 2
- Monitor for adverse effects including constipation (5%) and headache (3%) 2
- Fasting serum gastrin levels increase significantly (median 89% increase from baseline) but typically remain within normal range 2, 4
- Consider risk of lower respiratory tract infections, gastroenteritis, and candidemia with acid suppression 1
Long-term Use Warning
While studies have reported PPI use for up to 11 years in small numbers of children, enterochromaffin cell hyperplasia has been demonstrated in up to 50% of children receiving PPIs for more than 2.5 years 1. Limit treatment to the shortest effective duration.
Administration Timing
Administer once daily in the morning before meals for optimal absorption, as food reduces Cmax and AUC by 50-70% when given 30 minutes after food 2.
