Bevacizumab Dosing for Metastatic Colorectal Cancer

For patients receiving FOLFOX or FOLFIRI (every 2 weeks), administer bevacizumab 5 mg/kg IV on day 1; for patients receiving CAPOX (every 3 weeks), administer bevacizumab 7.5 mg/kg IV on day 1. 1

Dosing by Chemotherapy Regimen

Every 2-Week Regimens (FOLFOX/FOLFIRI)

  • Bevacizumab dose: 5 mg/kg IV on day 1
  • Repeat every 2 weeks 2, 1
  • This applies to:
    • mFOLFOX6 + bevacizumab
    • FOLFIRI + bevacizumab

Every 3-Week Regimens (CAPOX)

  • Bevacizumab dose: 7.5 mg/kg IV on day 1
  • Repeat every 3 weeks 2, 1
  • This applies to:
    • CapeOX (capecitabine + oxaliplatin) + bevacizumab

Administration Details

Infusion rate: Bevacizumab may be safely administered at 0.5 mg/kg/min, which translates to:

  • 5 mg/kg over 10 minutes
  • 7.5 mg/kg over 15 minutes 2

Timing: Administer bevacizumab on the same day as chemotherapy, typically after the chemotherapy infusion has been initiated 1.

Second-Line Setting

For patients progressing on first-line bevacizumab-containing therapy, the FDA-approved dosing is:

  • 5 mg/kg IV every 2 weeks with fluoropyrimidine-irinotecan or fluoropyrimidine-oxaliplatin regimens
  • 7.5 mg/kg IV every 3 weeks with fluoropyrimidine-irinotecan or fluoropyrimidine-oxaliplatin regimens 1

Research evidence supports that the 5 mg/kg dose every 2 weeks is appropriate for second-line therapy, with no additional benefit demonstrated from dose escalation to 10 mg/kg 3.

Critical Surgical Considerations

Withhold bevacizumab at least 28 days prior to elective surgery and do not resume until at least 28 days post-surgery with adequate wound healing 1. This is essential to minimize wound healing complications and surgical bleeding risk.

Important Caveats

  • The dose is weight-based (mg/kg), not body surface area-based
  • The dosing interval must match the chemotherapy schedule (every 2 weeks vs. every 3 weeks)
  • Do not reduce bevacizumab dose for toxicity; instead, withhold or discontinue based on specific adverse events 1
  • For patients with recent hemoptysis (≥2.5 mL), withhold bevacizumab 1
