Evaluation and Management of Infection and Retained Products After Induced Abortion

Ultrasound is the primary diagnostic tool for evaluating retained products of conception (RPOC) after induced abortion, with endometrial thickness ≥15mm with vascularity being the most reliable indicator, though diagnosis should not rely on imaging alone in asymptomatic patients. 1

Diagnostic Approach

Clinical Assessment Takes Priority

The diagnosis of RPOC should be based primarily on clinical symptoms rather than ultrasound findings alone. Asymptomatic patients with abnormal ultrasound findings typically do not require intervention, as these findings often resolve spontaneously 2.

Key clinical indicators of RPOC:

  • Persistent or heavy vaginal bleeding beyond 7 days
  • Pelvic pain or cramping
  • Fever (though transient fever within hours of misoprostol is normal and not concerning)
  • Signs of infection or sepsis

Ultrasound Criteria

When ultrasound is performed, the preferred terminology is now "incomplete early pregnancy loss" or "retained (or residual) products of conception" rather than outdated terms 1, 3.

Ultrasound findings suggestive of RPOC:

  • Endometrial thickness ≥15mm (most commonly used threshold) 4
  • Intracavitary tissue with internal vascularity on color Doppler 1
  • Persistent gestational sac 1

Critical caveat: Endometrial thickness <10mm has high negative predictive value for RPOC, while vascular flow in the endometrial cavity increases likelihood of true retained tissue 1. However, ultrasound alone has limited diagnostic accuracy - one study showed only 79% sensitivity even in symptomatic patients 5.

Distinguishing Infection from Normal Post-Procedure Findings

Fever evaluation:

  • Transient fever (5-10% incidence) resolving within hours after stopping misoprostol is normal 6
  • Fever persisting several hours after misoprostol cessation warrants evaluation for infection 6
  • Maintain high index of suspicion given morbidity of untreated septic abortion 7

Enhanced myometrial vascularity is a normal, transient finding after incomplete or completed early pregnancy loss and should not be confused with arteriovenous fistula or malformation 1, 3.

Management Algorithm

For Asymptomatic Patients with Ultrasound Findings

Expectant management is appropriate - up to 60% of women with suspected RPOC can avoid surgical intervention with expectant management over 8 weeks 8. Serial ultrasounds every 2 weeks until normal endometrial cavity is visualized 8, 2.

For Symptomatic Patients

1. If infection is suspected:

  • Rapid administration of broad-spectrum intravenous antibiotics targeting intrauterine source 7
  • Prompt uterine evacuation under antibiotic cover is the mainstay of treatment 9, 7
  • Engage critical care and OB-GYN specialists immediately 7

2. If hemorrhage without infection:

  • Curettage for RPOC is the tailored intervention 9
  • Consider uterine artery embolization if conservative measures fail 9

3. Medical management option: There is no clinically meaningful advantage for misoprostol treatment (800μg sublingual) compared to expectant management in women with suspected RPOC after first-trimester medical abortion 8. Both approaches succeed in approximately 60% of cases.

Infection Prevention

Universal antibiotic prophylaxis is NOT recommended for medication abortion given low infection rates and high number needed to treat 10.

For procedural abortion:

  • Universal antibiotic prophylaxis IS recommended across all gestational ages 10
  • Single dose doxycycline 200mg PO or azithromycin 500mg PO before procedure 10
  • Initiate before instrumentation for maximum efficacy 10

Common Pitfalls to Avoid

  1. Do not diagnose RPOC based solely on ultrasound in asymptomatic patients - abnormal findings often resolve without intervention 2

  2. Do not confuse transient misoprostol-related fever with infection - wait several hours after stopping misoprostol before attributing fever to infection 6

  3. Do not mistake enhanced myometrial vascularity for pathologic vascular anomaly - this is typically physiologic and transient 1, 3

  4. Do not delay treatment while awaiting STI results - test and treat for gonorrhea/chlamydia but do not postpone abortion 10

  5. Endometrial thickness ≥10mm in symptomatic women has 75% sensitivity and 100% specificity for RPOC, making it useful when positive but not reliable when negative 2
