Emtricitabine Requires Renal Dose Adjustment in Biktarvy

Emtricitabine is the component of Biktarvy that requires renal dose adjustment when creatinine clearance falls below 50 mL/min. However, because Biktarvy is a fixed-dose combination tablet, it cannot be dose-adjusted and should not be used when the estimated glomerular filtration rate (eGFR) is below 30 mL/min 1.

Understanding the Components

Biktarvy contains three antiretroviral drugs:

  • Bictegravir (integrase inhibitor) - no renal dose adjustment needed
  • Emtricitabine (nucleoside reverse transcriptase inhibitor) - requires dose adjustment with renal impairment
  • Tenofovir alafenamide (nucleotide reverse transcriptase inhibitor) - minimal renal adjustment needed

Renal Dosing Requirements by Component

Emtricitabine (the problematic component)

When used as a single agent, emtricitabine requires the following adjustments 2, 3:

  • CrCl ≥50 mL/min: 200 mg every 24 hours (standard dose)
  • CrCl 30-49 mL/min: 200 mg every 48 hours
  • CrCl 15-29 mL/min: 200 mg every 72 hours
  • CrCl <15 mL/min: 200 mg every 96 hours

Tenofovir Alafenamide

TAF has significantly better renal safety than its predecessor tenofovir disoproxil fumarate (TDF). TAF can be used when creatinine clearance is above 30 mL/min without dose adjustment 4, 1. This represents a major advantage over TDF, which requires dose adjustment when CrCl falls below 50 mL/min 2, 3.

Bictegravir

No renal dose adjustment is required for bictegravir 5.

Clinical Implications for Biktarvy Use

Biktarvy as a fixed-dose combination can be used when eGFR ≥30 mL/min without modification 1. The fixed-dose formulation prevents individual component dose adjustment, which is why it's contraindicated below this threshold.

Special Population: End-Stage Renal Disease on Hemodialysis

Recent evidence demonstrates that Biktarvy can be used in patients with end-stage renal disease (ESRD) on chronic hemodialysis 6, 7, 8. A phase 3b study showed:

  • Virologic suppression maintained in all participants over 48 weeks
  • Well tolerated with no treatment discontinuations
  • Bictegravir trough concentrations remained 4-7 fold higher than the effective concentration despite being lower than in patients with normal renal function 7

This represents an important advancement, as it allows once-daily dosing without the complexity of dose-adjusted regimens previously required for this population.

Common Pitfall to Avoid

Do not attempt to "dose adjust" Biktarvy by altering the dosing interval. The fixed-dose combination cannot be modified. If a patient's renal function declines below eGFR 30 mL/min (and they are not on chronic hemodialysis), you must switch to individual antiretroviral components that can be appropriately dose-adjusted, or select an alternative regimen 1.

The exception is patients on chronic hemodialysis, where once-daily Biktarvy has been validated as safe and effective 6, 7.
