When to Commence Empagliflozin in Chronic Kidney Disease

Initiate empagliflozin when eGFR is ≥20 mL/min/1.73 m² in patients with CKD, regardless of diabetes status, and continue it even if eGFR subsequently falls below 20 mL/min/1.73 m² unless dialysis is started.

eGFR-Based Initiation Criteria

The most recent KDIGO 2024 guidelines provide clear eGFR thresholds for starting empagliflozin 1:

  • Start empagliflozin at eGFR ≥20 mL/min/1.73 m² (1A recommendation)
  • Do NOT initiate if eGFR <20 mL/min/1.73 m²
  • Once started, continue empagliflozin even if eGFR drops below 20 mL/min/1.73 m², unless dialysis begins or the drug is not tolerated

This represents a significant departure from the FDA label 2, which states not to initiate empagliflozin if eGFR is <45 mL/min/1.73 m². The guideline evidence supersedes the FDA label based on the EMPA-KIDNEY trial 3, which demonstrated efficacy and safety down to eGFR 20 mL/min/1.73 m².

Clinical Indications by Patient Type

Type 2 Diabetes with CKD

Initiate empagliflozin in ALL patients with:

  • Type 2 diabetes AND
  • CKD with eGFR ≥20 mL/min/1.73 m²

This is a 1A recommendation regardless of albuminuria level 1.

CKD Without Diabetes

Initiate empagliflozin if eGFR ≥20 mL/min/1.73 m² AND any of the following:

  1. Urine albumin-to-creatinine ratio (ACR) ≥200 mg/g (≥20 mg/mmol) - 1A recommendation 1
  2. Heart failure (any ejection fraction) - 1A recommendation, irrespective of albuminuria 1
  3. eGFR 20-45 mL/min/1.73 m² with ACR <200 mg/g - 2B recommendation (weaker evidence but still suggested) 1

Key Point on Albuminuria

The EMPA-KIDNEY trial 4 demonstrated that empagliflozin slowed CKD progression across all albuminuria levels, with an 86% relative reduction in chronic eGFR slope in patients with ACR <30 mg/g. Do not withhold empagliflozin based solely on low albuminuria if other criteria are met.

Absolute Contraindications

Do NOT initiate empagliflozin in:

  • End-stage renal disease (ESKD) on dialysis 2
  • Severe hypersensitivity to empagliflozin 2

Note: While one small safety study 5 explored empagliflozin in hemodialysis patients, the FDA label and guidelines clearly contraindicate use in dialysis patients. The guideline states to discontinue when kidney replacement therapy (KRT) is initiated 1.

Temporary Withholding Situations

Temporarily hold empagliflozin during:

  • Prolonged fasting
  • Surgery
  • Critical medical illness (increased ketoacidosis risk) 1
  • Volume depletion states until corrected 2

Resume once the acute situation resolves.

Monitoring After Initiation

Empagliflozin does NOT require increased monitoring frequency 1. The initial eGFR dip (typically 2-6% in first 2 months 4) is expected, reversible, and not an indication to stop therapy 1.

Standard CKD monitoring intervals remain appropriate:

  • Check renal function, electrolytes, and volume status at baseline
  • The acute eGFR decline is hemodynamic and expected
  • Continue routine CKD follow-up per usual practice

Practical Algorithm

Step 1: Assess eGFR

  • eGFR ≥20 mL/min/1.73 m² → Proceed to Step 2
  • eGFR <20 mL/min/1.73 m² → Do NOT initiate (unless already on therapy)
  • On dialysis → Contraindicated

Step 2: Check for diabetes

  • Type 2 diabetes present → START empagliflozin (1A)
  • No diabetes → Proceed to Step 3

Step 3: Assess albuminuria and heart failure

  • ACR ≥200 mg/g OR heart failure → START empagliflozin (1A)
  • ACR <200 mg/g, no heart failure, eGFR 20-45 → Consider starting empagliflozin (2B)
  • ACR <200 mg/g, no heart failure, eGFR >45 → Weaker indication (individualize)

Step 4: Exclude contraindications

  • Check for dialysis status and drug allergies
  • Assess volume status and correct if depleted before starting

Common Pitfalls to Avoid

  1. Don't stop empagliflozin when eGFR drops initially - The acute dip is expected and beneficial long-term 4
  2. Don't withhold based on FDA label eGFR cutoff of 45 - Guidelines based on EMPA-KIDNEY support initiation at eGFR ≥20 1
  3. Don't require high albuminuria to start - Benefits extend to patients with minimal albuminuria 4
  4. Don't discontinue when eGFR falls below 20 during treatment - Continue until dialysis starts 1
  5. Don't forget to temporarily hold during surgery/critical illness - Resume after recovery 1

Combination Therapy Considerations

Empagliflozin can and should be combined with:

  • RAS inhibitors (ACEi/ARB) - Continue both agents 1
  • Nonsteroidal MRA (finerenone) - Can be added for additional benefit in type 2 diabetes with persistent albuminuria 1, 6

The combination of empagliflozin with finerenone showed greater albuminuria reduction than either agent alone without unexpected safety signals 6.
