Contraindications to Albuterol Use

The only absolute contraindication to albuterol is a history of hypersensitivity to albuterol or any of its components 1.

Absolute Contraindication

  • Hypersensitivity reactions: Patients with documented allergy to albuterol or any component of the formulation should not receive this medication 1. Rare cases of immediate hypersensitivity reactions including urticaria, angioedema, rash, bronchospasm, and oropharyngeal edema have been reported 1.

Critical Precautions (Use with Extreme Caution)

While not absolute contraindications, the following conditions require extreme caution and may represent relative contraindications depending on clinical context:

Cardiovascular Conditions

Albuterol should be used with extreme caution in patients with:

  • Coronary insufficiency (active cardiac ischemia)
  • Cardiac arrhythmias (particularly tachyarrhythmias)
  • Hypertension (especially uncontrolled)
  • Advanced aortic stenosis 2

The β2-agonist effects can cause tachycardia, increased cardiac output, decreased systemic vascular resistance, and potential myocardial ischemia 3, 1. Research demonstrates that even inhaled albuterol increases heart rate, cardiac output, and stroke volume while decreasing systemic vascular resistance 4. In patients with underlying cardiac disease, these hemodynamic changes may precipitate acute coronary syndromes or heart failure decompensation 5.

Concurrent β-Blocker Therapy

Albuterol is contraindicated in patients receiving concurrent β-blocker therapy 2. β-blockers and albuterol have mutually inhibitory effects 1. In patients on β-blockers, albuterol may paradoxically worsen bronchospasm through unopposed α-adrenergic effects and can cause refractory hypotension 6. This is particularly dangerous in anaphylaxis scenarios where β-blocker therapy can make patients resistant to epinephrine and albuterol treatment.

Monoamine Oxidase Inhibitors (MAOIs) and Tricyclic Antidepressants

Extreme caution is required when administering albuterol to patients on MAOIs or tricyclic antidepressants, as the vascular effects of albuterol may be significantly potentiated 1.

Other High-Risk Conditions Requiring Caution

  • Convulsive disorders: Sympathomimetic stimulation may lower seizure threshold 1
  • Hyperthyroidism: Amplifies sympathomimetic effects 1
  • Diabetes mellitus: Large doses can aggravate pre-existing diabetes and ketoacidosis; may mask hypoglycemia symptoms 7, 1
  • Patients unusually responsive to sympathomimetic amines 1

Important Clinical Considerations

Paradoxical Bronchospasm

Albuterol can produce life-threatening paradoxical bronchospasm 1. If this occurs, discontinue immediately and institute alternative therapy. This is a medical emergency requiring immediate recognition and treatment change.

Hypokalemia Risk

Albuterol may produce significant hypokalemia through intracellular potassium shunting, with potential for adverse cardiovascular effects 1. Studies show decreases of 20-25% in serum potassium levels in children, and mean decreases of 0.52-0.54 mmol/L in adults 8. While usually transient and asymptomatic, this can be dangerous in patients with cardiac disease or those on medications affecting potassium balance.

Pregnancy

Albuterol is Pregnancy Category C. It has shown teratogenic effects in mice (cleft palate at 1.25 times human dose) and cranioschisis in rabbits at high doses 1. Use only if potential benefit justifies potential fetal risk.

Key Pitfall to Avoid

Do not confuse relative precautions with absolute contraindications. The only true contraindication is hypersensitivity 1. Even in patients with cardiovascular disease, the benefit of treating severe bronchospasm typically outweighs risks, but requires careful monitoring and dose titration. The evidence suggests that acute administration of inhaled β2-agonists, even in heart failure patients, may improve pulmonary function and hemodynamics without significant increase in malignant dysrhythmias 9.
