Follow-up β-hCG After Medical Termination
Check serum β-hCG 7 days after mifepristone administration, with successful abortion confirmed by an ≥80% decline from baseline or an absolute level <900-1000 IU/L at 14-21 days post-abortion.
Timing and Interpretation of β-hCG Testing
Early Follow-up (Day 5-7)
For patients requiring early confirmation of complete abortion, a single β-hCG measurement on day 5 after mifepristone shows a predictable decline of approximately 91% in successful medical abortions 1. The most robust recent data demonstrates that an ≥80% decline in β-hCG within 7 days (±2 days) post-mifepristone effectively confirms complete abortion with 96.8% accuracy 2. This threshold reliably excludes both ectopic pregnancy and ongoing pregnancy, as neither of these conditions demonstrates an ≥80% decline 2.
Key interpretation points for day 5-7 testing:
- Mean decline for successful abortion: 70% at day 3,91% at day 5 1
- No ectopic pregnancies show ≥50% decline by days 3-5 3
- Rising or insufficient decline (<80%) indicates ectopic pregnancy, ongoing pregnancy, or incomplete abortion requiring further evaluation
Standard Follow-up (Day 14-21)
The optimal absolute β-hCG threshold at 14-21 days post-abortion is <900 IU/L to exclude ongoing pregnancy with 100% sensitivity and 81.5% specificity 4. While some studies suggest a threshold of 1000 IU/L (85.7% sensitivity, 83.5% specificity) 4, or 253 IU/L for 95% exclusion of failure 5, the <900 IU/L cutoff provides the highest sensitivity for ruling out ongoing pregnancy.
Clinical Algorithm
Day 7 (±2 days) post-mifepristone:
- Measure serum β-hCG
- If decline ≥80% from baseline → Complete abortion confirmed
- If decline <80% or rising → Proceed to ultrasound evaluation for ectopic, ongoing, or incomplete abortion
Day 14-21 post-mifepristone (alternative or confirmatory):
- Measure serum β-hCG
- If <900 IU/L → Complete abortion confirmed
- If ≥900 IU/L → Perform ultrasound to assess for ongoing pregnancy
Important Caveats
β-hCG trends cannot distinguish between ectopic pregnancy and retained intrauterine pregnancy 3, 2. When β-hCG fails to decline appropriately, ultrasound examination becomes mandatory to differentiate these conditions and guide management.
The rate of β-hCG decline is not affected by initial β-hCG level or gestational duration through 63 days gestation 1, making these thresholds applicable across the first trimester.
Unplanned intervention rates are low (6-8%) regardless of follow-up method 6, and both β-hCG and ultrasound follow-up demonstrate high patient satisfaction (88-95%) 6.
For patients with pregnancy of unknown location who receive same-day medication abortion, serial β-hCG testing is effective for confirming successful abortion and identifying those requiring further follow-up 3, though it cannot definitively exclude ectopic pregnancy without imaging when levels fail to decline appropriately.
