FDA Black Box Warnings
Candesartan
Candesartan carries an FDA black box warning for fetal toxicity—when pregnancy is detected, discontinue immediately. 1
The warning specifically states:
- Drugs that act directly on the renin-angiotensin system can cause injury and death to the developing fetus 1
- Discontinuation should occur as soon as pregnancy is detected 1
Clinical Context and Contraindications
Multiple guidelines reinforce this critical warning across different clinical scenarios:
- Pregnancy is an absolute contraindication for candesartan and all angiotensin receptor blockers (ARBs) 2, 3, 4
- During pregnancy, ACE inhibitors, ARBs, direct renin inhibitors, and spironolactone are all contraindicated due to potential fetal damage 3
- The use of these agents in pregnancy is associated with adverse fetal effects including neonatal acute kidney injury, lung toxicity, skull hypoplasia, and congenital malformations of the cardiovascular system, central nervous system, and kidneys 5
Special Considerations for Women of Childbearing Potential
Women intending to become pregnant should switch from candesartan to an alternative antihypertensive medication approved during pregnancy before conception 3. Safe alternatives during pregnancy include methyldopa, labetalol, and long-acting nifedipine 4.
For migraine prevention specifically, candesartan is explicitly contraindicated in pregnancy and should be avoided, with propranolol being the recommended first choice if preventive medication is required 2.
Adderall (Mixed Amphetamine Salts)
Adderall does NOT carry an FDA black box warning. However, it does carry multiple serious warnings in the precautions section that warrant careful clinical attention.
Key FDA Warnings (Not Black Box)
The most significant warnings for Adderall include:
1. Serious Cardiac Risk:
- Potential risks exist for patients with serious cardiac disease, including sudden death 6
- Patients should contact healthcare providers immediately if they develop exertional chest pain, unexplained syncope, or other cardiac symptoms 6
- Amphetamines increase systolic blood pressure by approximately 1.93 mmHg and diastolic blood pressure by 1.84 mmHg, with heart rate increasing by 3.71 beats per minute 7
2. Abuse, Misuse, and Addiction:
- Risk of abuse, misuse, and addiction that can lead to overdose and death 6
- Patients should store Adderall in a safe, preferably locked location 6
3. Psychiatric Adverse Reactions:
- Can cause psychotic or manic symptoms at recommended doses, even in patients without prior psychiatric history 6
4. Peripheral Vasculopathy:
- Risk of circulation problems in fingers and toes, including Raynaud's phenomenon 6
- Fingers or toes may feel numb, cool, painful, or change color from pale to blue to red 6
5. Serotonin Syndrome:
- Risk when used concomitantly with other serotonergic drugs (SSRIs, SNRIs, triptans, MAOIs, etc.) 6
6. Growth Suppression:
- Can cause slowing of growth including weight loss in pediatric patients 6
Pregnancy and Lactation Considerations
Unlike candesartan, amphetamines do not have a black box warning for pregnancy:
- Overall, amphetamines do not appear to be associated with major congenital malformations or cardiac malformations 8
- Possible small increased risks for gastroschisis (aOR 3.0), preeclampsia (aRR 1.29), and preterm birth (aRR 1.30) when use continues in second half of pregnancy, though absolute risks remain small 8
- Breastfeeding does not appear to adversely affect infants, though monitoring for irritability, insomnia, and feeding difficulty is recommended 8
- Discontinuing treatment during pregnancy can lead to worse maternal mental health outcomes and increased risks for spontaneous abortion and preterm birth from untreated ADHD 8
Critical Drug Interactions
Absolute contraindication: Concomitant use with MAOIs or within 14 days of discontinuing MAOIs can cause hypertensive crisis with potential outcomes including death, stroke, myocardial infarction, and aortic dissection 6.
