Cetirizine Dosing for a 15-Month-Old Child
Cetirizine is not FDA-approved for children under 6 years of age, and the available FDA drug labeling explicitly states to "ask a doctor" for children under 6 years 1. However, based on high-quality pediatric research evidence, cetirizine can be safely administered to infants as young as 6 months at a dose of 0.25 mg/kg twice daily (approximately 2.5 mg twice daily for most 15-month-olds).
Evidence-Based Dosing
The most robust evidence comes from the ETAC (Early Treatment of the Atopic Child) study, which was the largest and longest randomized, double-blind, placebo-controlled safety investigation of any H1-antihistamine ever conducted in children 2. This landmark study enrolled 817 infants aged 12-24 months and demonstrated:
- Dose: 0.25 mg/kg twice daily (mean daily dose 4.5 mg)
- Duration studied: 18 months of continuous use
- Safety profile: Equivalent to placebo with no clinically relevant differences in neurologic or cardiovascular symptoms, growth, development, or laboratory parameters
- No QTc prolongation observed on electrocardiograms
Practical Application for a 15-Month-Old
For a typical 15-month-old weighing approximately 10-11 kg:
- Calculate dose: 0.25 mg/kg × 10 kg = 2.5 mg per dose
- Frequency: Twice daily (morning and evening)
- Total daily dose: 5 mg/day
This dosing has been validated in multiple studies 3, 4, 5 specifically in the 6-11 month and 12-24 month age groups.
Important Clinical Considerations
Pharmacokinetics in infants differ from adults:
- Elimination half-life in infants is shorter (3.1 hours) compared to adults 5
- Despite shorter half-life, pharmacodynamic effects (histamine wheal/flare inhibition) persist for 12 hours, supporting twice-daily dosing 5
- Renal excretion accounts for 62.7% of the dose 5
Safety profile in this age group:
- The short-term safety study in 6-11 month-olds showed no difference in adverse events between cetirizine and placebo 3
- A trend toward fewer sleep disturbances was observed in the cetirizine group compared to placebo 3
- Somnolence rates are low (0-1.3%) and comparable to placebo 3, 2
Critical Caveats
Off-label use: Since FDA approval is only for ages 6 years and older 1, prescribing for a 15-month-old constitutes off-label use. This requires:
- Informed discussion with parents/caregivers about off-label status
- Documentation of the risk-benefit assessment
- Consideration of the specific indication (atopic dermatitis, urticaria, allergic rhinitis)
Renal impairment: If the child has any renal dysfunction, the dose should be reduced by 50% 6.
Formulation: Use cetirizine oral solution/syrup for accurate dosing in this age group, not tablets.
Duration: While the ETAC study demonstrated safety for 18 months of continuous use 4, 2, treatment duration should be based on clinical need and regular reassessment.
