Potent steroid creams, such as clobetasol propionate 0.05%, should be used cautiously on the arm, with an initial dose of 30-40g per day, and tapered according to a specific schedule to minimize side effects and prevent rebound phenomena, as recommended by the European Dermatology Forum consensus 1. When using potent steroid creams on the arm, it is essential to follow a tapering schedule to reduce the risk of side effects, such as skin thinning, striae, and skin color changes.
- The initial dose of clobetasol propionate 0.05% should be 30-40g per day, applied in two applications over the entire body, including both normal skin and blisters and erosions, but sparing the face 1.
- The dose should be reduced 15 days after disease control, and a tapering schedule with dose adaptation should be followed, with daily treatment in the first month, treatment every 2 days in the second month, treatment twice per week in the third month, and treatment once per week starting in the fourth month 1.

DOSAGE AND ADMINISTRATION: Apply a thin layer of clobetasol propionate gel, cream or ointment to the affected skin areas twice daily and rub in gently and completely. Clobetasol propionate gel, cream and ointment are super-high potency topical corticosteroids; therefore, treatment should be limited to 2 consecutive weeks, and amounts greater than 50 g per week should not be used. INDICATIONS AND USAGE: Clobetasol propionate gel, cream and ointment are super-high potency corticosteroid formulations indicated for the relief of the inflammatory and pruritic manifestations of corticosteroid responsive dermatoses. Treatment beyond 2 consecutive weeks is not recommended, and the total dosage should not exceed 50 g per week because of the potential for the drug to suppress the hypothalamic-pituitary-adrenal (HPA) axis.
