From the Guidelines
For a 15-year-old with otitis externa (swimmer's ear), I recommend treatment with topical antibiotic ear drops containing ciprofloxacin 0.3% and dexamethasone 0.1% (such as Ciprodex), 4 drops into the affected ear twice daily for 7 days. Before applying drops, gently clean the ear canal with a cotton swab if visible debris is present. For pain management, acetaminophen 650 mg every 6 hours or ibuprofen 400 mg every 6 hours as needed can be used 1. The ear should be kept dry during treatment; avoid swimming and use earplugs or cotton balls coated with petroleum jelly when showering. If the ear canal is significantly swollen, an ear wick may need to be inserted by a healthcare provider to facilitate medication delivery. Treatment should continue for the full 7 days even if symptoms improve earlier. This regimen is effective because the antibiotic component addresses bacterial infection while the steroid component reduces inflammation and swelling. If symptoms worsen or don't improve after 48-72 hours of treatment, or if fever develops, the patient should return for reevaluation as this may indicate a more serious infection requiring different management.
Some key points to consider when treating otitis externa include:
- The importance of keeping the ear dry during treatment to prevent further irritation and infection 1
- The use of pain management medications such as acetaminophen or ibuprofen to help alleviate discomfort 1
- The potential need for an ear wick to be inserted by a healthcare provider if the ear canal is significantly swollen 1
- The importance of completing the full 7-day course of treatment even if symptoms improve earlier to prevent relapse of infection 1
It's also important to note that patients with certain underlying conditions, such as diabetes or immunocompromised states, may require special consideration and potentially different management strategies 1. Additionally, the use of aural toilet and irrigation of the ear canal should be approached with caution in certain patients, such as those with diabetes or immunocompromised states, due to the risk of necrotizing otitis externa 1.
From the FDA Drug Label
For the Treatment of Acute Otitis Externa (age 6 months and older) The recommended dosage regimen is as follows: Four drops [equivalent to 0. 14 mL of ciprofloxacin and dexamethasone otic suspension, (consisting of 0.42 mg ciprofloxacin and 0. 14 mg dexamethasone)] instilled into the affected ear twice daily for seven days. For pediatric patients (from 6 months to 13 years old): Five drops (0.25 mL, 0.75 mg ofloxacin) instilled into the affected ear once daily for seven days. For patients 13 years and older: Ten drops (0.5 mL, 1. 5 mg ofloxacin) instilled into the affected ear once daily for seven days.
The treatment guidelines for otitis externa in a 15-year-old patient are as follows:
- Medications:
- Ciprofloxacin and dexamethasone otic suspension
- Ofloxacin otic solution
- Concentration of solution:
- Ciprofloxacin and dexamethasone otic suspension: 0.3% ciprofloxacin and 0.1% dexamethasone
- Ofloxacin otic solution: 0.3% ofloxacin
- Dosage of medication:
- Ciprofloxacin and dexamethasone otic suspension: Four drops (0.14 mL) twice daily for seven days
- Ofloxacin otic solution: Ten drops (0.5 mL) once daily for seven days
- Length of time symptoms: Treatment should be continued for seven days 2, 2, 3.
From the Research
Treatment Guidelines for Otitis Externa
Medications and Concentration of Solution
- Ciprofloxacin 0.3%/dexamethasone 0.1% (CIP/DEX) otic suspension is a commonly used medication for otitis externa 4, 5, 6
- Neomycin 0.35%/polymyxin B 10,000 IU/mL/hydrocortisone 1.0% (N/P/H) otic suspension is another option 4, 5, 6
- Ciprofloxacin (2 mg/ml) as ear drops twice daily is also effective 7
- Oxytetracycline (5 mg/ml) polymyxin B (10,000 units/ml) and hydrocortisone (15 mg/ml) twice daily for 7 days is another treatment option 7
Dosage of Medication
- CIP/DEX: 3-4 drops twice daily for 7 days 4, 5
- N/P/H: 3-4 drops three times daily for 7 days 4, 5
- Ciprofloxacin: 2 mg/ml as ear drops twice daily 7
- Oxytetracycline, polymyxin B, and hydrocortisone: twice daily for 7 days 7
Length of Time Symptoms
- Symptoms of otitis externa can last for several days to a week or more without treatment
- With treatment, symptoms can improve within 3-4 days 5
- Treatment failure rates for Pseudomonas aeruginosa and Staphylococcus aureus can vary depending on the medication used 6
Effectiveness of Treatments
- CIP/DEX is clinically and microbiologically superior to N/P/H in the treatment of mild to severe otitis externa 4
- Ciprofloxacin is effective in eradicating bacterial pathogens, including Pseudomonas aeruginosa and Staphylococcus aureus 7, 6
- Antiseptic, steroid, and antibiotic monotherapies are all effective for the management of acute otitis externa, with no significant differences in cure rates 8
