From the Guidelines
For a 15-year-old with otitis externa (swimmer's ear), I recommend treatment with topical antibiotic ear drops containing ciprofloxacin 0.3% and dexamethasone 0.1% (such as Ciprodex), 4 drops into the affected ear twice daily for 7 days, as this treatment has been shown to be effective in managing the condition with minimal side effects 1. The patient should keep the ear dry during treatment by avoiding swimming and using a cotton ball coated with petroleum jelly during showers.
- Symptoms of otitis externa include:
- Ear pain (often severe)
- Itching in the ear canal
- Redness and swelling of the ear canal
- Drainage from the ear (often yellow or green)
- Temporary hearing loss
- Tenderness when touching the ear or jaw Pain relief can be achieved with acetaminophen or ibuprofen as needed.
- If symptoms worsen or don't improve after 48-72 hours of treatment, the patient should be reevaluated, as most people feel better within 48 to 72 hours and have minimal or no symptoms by 7 days 1. Topical treatments are preferred over oral antibiotics because they deliver medication directly to the infection site with fewer systemic side effects, and the acidic environment created by these solutions helps inhibit bacterial growth in the ear canal.
- It is essential to note that eardrops alone are the most effective treatment for AOE and may contain antibiotics, antiseptics, steroids, or a combination, and antibiotics taken by mouth do not kill most germs that cause AOE and should be used only when infection spreads beyond the ear canal, eardrops cannot get into the ear, or the immune system is weak 1. Before applying drops, the ear canal should be gently cleaned of debris if possible, and the patient should be advised to avoid scratching or touching the ear and not insert anything into the ear canal, including cotton-tipped swabs, to minimize the risk of further irritation or infection 1.
- The patient should also be informed that eardrops are, in general, very safe and well tolerated, but some people may report local rash, itching, irritation, or discomfort, and if they taste the eardrops, it means there is likely a hole or perforation of the eardrum, so they should inform their doctor (if they have not already done so) 1.
From the FDA Drug Label
For the Treatment of Acute Otitis Externa (age 6 months and older) The recommended dosage regimen is as follows: Four drops [equivalent to 0. 14 mL of ciprofloxacin and dexamethasone otic suspension, (consisting of 0.42 mg ciprofloxacin and 0. 14 mg dexamethasone)] instilled into the affected ear twice daily for seven days. Contents of one single-dose container should be instilled into the affected ear twice daily (approximately 12 hours apart) for 7 days.
The treatment guidelines for otitis externa in a 15-year-old include:
- Medications:
- Ciprofloxacin and dexamethasone otic suspension
- Ciprofloxacin otic solution, 0.2%
- Concentration of solution:
- Ciprofloxacin and dexamethasone otic suspension: 0.42 mg ciprofloxacin and 0.14 mg dexamethasone per 0.14 mL
- Ciprofloxacin otic solution, 0.2%: 0.5 mg ciprofloxacin in 0.25 mL
- Dosage of medication:
- Four drops of ciprofloxacin and dexamethasone otic suspension twice daily
- One single-dose container of ciprofloxacin otic solution, 0.2% twice daily
- Length of time: 7 days
- Symptoms of otitis externa: not explicitly listed in the provided drug labels 2 3
From the Research
Treatment Guidelines for Otitis Externa
- The recommended treatment for otitis externa includes topical antibiotics and corticosteroids, such as ciprofloxacin 0.3%/dexamethasone 0.1% (CIP/DEX) or neomycin 0.35%/polymyxin B 10,000 IU/mL/hydrocortisone 1% (NPH) 4, 5.
- The concentration of the solution and dosage of medication may vary, but common regimens include CIP/DEX twice daily or NPH three times daily for 7 days 4, 5.
- The length of time for treatment is typically 7 days, but may be extended depending on the severity of the infection and the patient's response to treatment 4, 5.
Symptoms of Otitis Externa
- Symptoms of otitis externa may include ear pain, inflammation, tenderness, edema, and discharge 4, 5.
- Patients may also experience itching, redness, and swelling of the external auditory canal 6.
- In severe cases, otitis externa can lead to more serious complications, such as temporary hearing loss or cellulitis 7.
Medications and Concentrations
- Ciprofloxacin 0.3%/dexamethasone 0.1% (CIP/DEX) is a commonly used topical antibiotic and corticosteroid combination for the treatment of otitis externa 4, 5.
- Neomycin 0.35%/polymyxin B 10,000 IU/mL/hydrocortisone 1% (NPH) is another commonly used topical antibiotic and corticosteroid combination for the treatment of otitis externa 4, 5.
- Ciprofloxacin 0.2% solution is also effective in the treatment of otitis externa, with high in vitro activity against Pseudomonas aeruginosa 6.
