Xyrem Titration Dosing

Start Xyrem at 4.5 g per night (2.25 g at bedtime and 2.25 g taken 2.5-4 hours later), then increase by 1.5 g per night at weekly intervals until reaching the effective dosage range of 6-9 g per night. 1

Initial Dosing

The FDA-approved starting dose is 4.5 g per night divided into two equal doses:

  • 2.25 g at bedtime
  • 2.25 g taken 2.5-4 hours later 1

This applies to adults and pediatric patients ≥45 kg. For pediatric patients weighing less, weight-based dosing applies (see below).

Titration Schedule

Increase the dose by 1.5 g per night at weekly intervals (adding 0.75 g to each of the two nightly doses) 1. The standard titration progression is:

Week Total Nightly Dose Bedtime Dose Second Dose (2.5-4 hrs later)
1 4.5 g 2.25 g 2.25 g
2 6 g 3 g 3 g
3 7.5 g 3.75 g 3.75 g
4 9 g 4.5 g 4.5 g

Target Dosing Range

The recommended effective dosage range is 6-9 g per night 1. Research demonstrates dose-dependent improvements in both cataplexy and excessive daytime sleepiness, with the 9 g dose showing the most robust efficacy 2, 3. In clinical trials, weekly titration was better tolerated than fixed-dose initiation, with fewer adverse events 4.

Pediatric Dosing (7 Years and Older)

Weight-based dosing applies 1:

  • 20-30 kg: Start ≤1 g per dose (≤2 g total nightly); increase by 0.5 g per dose weekly; maximum 3 g per dose (6 g total nightly)
  • 30-45 kg: Start ≤1.5 g per dose (≤3 g total nightly); increase by 0.5 g per dose weekly; maximum 3.75 g per dose (7.5 g total nightly)
  • ≥45 kg: Follow adult dosing above

Critical Administration Requirements

Patients must take each dose while in bed and lie down immediately after dosing 1. Key safety considerations:

  • Wait at least 2 hours after eating before the first dose to optimize absorption and reduce nausea 1
  • Dilute each dose with approximately ¼ cup (60 mL) of water in pharmacy-provided containers 1
  • Prepare both doses before bedtime 1
  • Patients may fall asleep within 5-15 minutes of the first dose 5

Dose Optimization

While equal divided doses are standard, some patients achieve better responses with unequal doses at bedtime versus 2.5-4 hours later 1. Titrate based on efficacy and tolerability rather than rushing to maximum dose. In open-label studies where patients were titrated to effect, 85% completed treatment with final doses distributed across the range: 15% at 4.5 g, 40% at 6 g, 33% at 7.5 g, and 11% at 9 g 6.

Important Safety Warnings

Xyrem carries an FDA black box warning for CNS depression and respiratory depression 7, 1. It is a Schedule III controlled substance and the sodium salt of GHB (Schedule I). Avoid combining with alcohol or other CNS depressants 1. Monitor for:

  • Depression and suicidality (7% incidence in trials) 1
  • Confusion (3% overall, 17% at 9 g dose) 1
  • Parasomnias including sleepwalking (6% incidence) 1
  • Sleep-disordered breathing and oxygen desaturation 1

Special Populations

Hepatic impairment: Start at one-half the standard dose 1

High sodium sensitivity (heart failure, hypertension, renal impairment): Consider that 9 g nightly contains 1640 mg sodium 1. Low-sodium oxybate formulations (Xywav) contain 92% less sodium and may be preferred 8, 9.

Clinical Pearls

Doses above 9 g per night have not been studied and should not be administered 1. Most patients (90%) report symptom improvement by week 12, with 60% rating symptoms as "much improved" in a dose-dependent manner 6. The anticataplectic and alerting effects are incremental to concomitant stimulants 2.
