Heplisav-B Dosing Schedule
Heplisav-B is administered as a 2-dose series with each 0.5 mL dose given intramuscularly in the deltoid region at 0 and 1 month 1.
Standard Dosing Regimen
According to the 2022 ACIP guidelines and FDA labeling, the dosing schedule for Heplisav-B is straightforward:
- Dose: 20 μg in 0.5 mL per injection
- Route: Intramuscular injection in the deltoid muscle
- Schedule: Two doses separated by 1 month (at 0 and 1 month) 2, 1
- Age indication: Adults ≥18 years 2, 1
This represents a significant advantage over traditional 3-dose hepatitis B vaccines (Engerix-B, Recombivax HB) which require 6 months to complete the series at 0,1, and 6 months 2.
Key Clinical Considerations
When to Use Heplisav-B
The 2-dose schedule makes Heplisav-B particularly advantageous for:
- Patients with poor adherence to multi-visit schedules
- Healthcare workers needing rapid seroprotection
- Adults with diabetes or other at-risk conditions 2
Important Contraindications and Limitations
Do NOT use Heplisav-B in:
- Pregnant women - insufficient safety data; use Engerix-B, Recombivax HB, or Twinrix instead 2
- Adults on hemodialysis - safety and effectiveness not established 2
- Breastfeeding women - insufficient data on effects on infant or milk production 2
If Schedule is Interrupted
If the vaccination series is interrupted, do not restart the series 2. However, to achieve a valid 2-dose Heplisav-B series, both doses must be Heplisav-B with at least 4 weeks (1 month) between them 3. If only one dose of Heplisav-B was given and the series is being completed with a different vaccine, you must switch to a 3-dose schedule.
Vaccine Interchangeability
While the same manufacturer's vaccine is preferred to complete a series, vaccination should not be delayed if the previous vaccine type is unknown or unavailable 3. However, for Heplisav-B specifically, both doses must be Heplisav-B to constitute a valid 2-dose series 3.
Clinical Efficacy Notes
Research demonstrates that Heplisav-B achieves:
- Rapid seroprotection within 1 month after the second dose 4
- Higher seroprotection rates compared to traditional 3-dose vaccines 5, 6
- 84% response rate after just 1 dose and 91% after 2 doses in healthcare worker nonresponders to previous vaccines 5
For patients who fail to seroconvert (HBsAb <10 IU/L) after the standard 2-dose series, emerging evidence suggests a third Heplisav-B dose may achieve seroconversion in approximately 57% of nonresponders, particularly in immunocompromised populations like those with inflammatory bowel disease 7.
