Does Jardiance Improve LVEF?

Yes, Jardiance (empagliflozin) can improve LVEF in patients with heart failure and reduced ejection fraction (HFrEF), though this is not its primary mechanism of benefit and the magnitude of improvement is modest.

Evidence for LVEF Improvement in HFrEF

The strongest evidence comes from a 2021 randomized controlled trial specifically designed to assess structural cardiac changes 1. In this double-blind, placebo-controlled study of 84 nondiabetic HFrEF patients treated with empagliflozin 10 mg daily for 6 months, LVEF improved by 6.0 ± 4.2% in the empagliflozin group versus -0.1 ± 3.9% in placebo (p < 0.001). This was accompanied by significant reductions in left ventricular end-diastolic volume (-25.1 ml vs -1.5 ml, p < 0.001), end-systolic volume (-26.6 ml vs -0.5 ml, p < 0.001), and LV mass (-17.8 g vs 4.1 g, p < 0.001) 1.

However, a more recent 2025 trial (EMPRESS-MI) in post-MI patients with reduced LVEF showed no effect on LVEF or cardiac volumes when empagliflozin was started early after myocardial infarction 2. This suggests the benefit may be context-dependent and more pronounced in chronic stable HFrEF rather than acute post-MI settings.

Clinical Context: LVEF Improvement is Not the Primary Goal

The critical point is that LVEF improvement, while demonstrable, is not why we prescribe empagliflozin in heart failure. Current guidelines prioritize empagliflozin for its proven mortality and morbidity benefits, not for improving ejection fraction 3, 4.

For HFrEF (LVEF ≤40%):

  • Empagliflozin is a Class 2a recommendation to reduce cardiovascular death and heart failure hospitalizations 3
  • In EMPEROR-Reduced, empagliflozin reduced the composite of CV death or HF hospitalization by 25% (HR 0.75, p < 0.001) 4, 5
  • This benefit occurred regardless of diabetes status and was primarily driven by a 31% reduction in HF hospitalizations 4

For HFmrEF (LVEF 41-49%):

  • Empagliflozin is Class 2a for reducing HF hospitalizations and CV mortality 3
  • Post-hoc analysis from EMPEROR-Preserved showed benefit in this LVEF range 3

For HFpEF (LVEF >40%):

  • Empagliflozin is recommended (Strong Recommendation) to reduce HF hospitalizations 4
  • EMPEROR-Preserved showed 29% reduction in first HF hospitalization (HR 0.71, p < 0.001) 4
  • No significant LVEF improvement is expected or needed in this population 6

Practical Algorithm for Prescribing

Start empagliflozin 10 mg daily in:

  1. Any symptomatic HF patient with LVEF ≤40% on guideline-directed medical therapy (ACEi/ARB/ARNI, beta-blocker, MRA) 3, 4
  2. Symptomatic HF patients with LVEF 41-49%, especially those on the lower end of this spectrum 3
  3. Symptomatic HF patients with LVEF >40% to prevent hospitalizations 4

Key requirements:

  • eGFR ≥20 mL/min/1.73 m² 5
  • Stabilized patients (not for acute decompensation management) 4
  • Independent of diabetes status 4

Important Caveats

  • Don't chase LVEF improvement as a goal. The 6% absolute LVEF increase seen in research 1 is modest compared to the robust 25-31% reduction in hard clinical outcomes 4, 5
  • The FDA label for Jardiance does not list LVEF improvement as an indication 5
  • Recent acute echocardiographic studies show empagliflozin primarily affects left atrial volumes and filling parameters rather than immediate LVEF changes 7
  • In post-MI patients, contemporary standard care already prevents adverse remodeling in most patients, potentially limiting additional SGLT2i benefit on LVEF 2

Bottom line: Prescribe empagliflozin for its proven mortality and hospitalization benefits across the LVEF spectrum, not specifically to improve ejection fraction. Any LVEF improvement is a secondary benefit in chronic HFrEF patients.
