Antifungal Shampoo for Tinea Capitis
Use ketoconazole 2% or selenium sulfide 1% shampoo as adjunctive therapy alongside oral antifungals—shampoo alone will NOT cure tinea capitis but reduces spore transmission to others. 1
Critical Understanding: Shampoo is NOT Primary Treatment
The British Association of Dermatologists guidelines are unequivocal: topical therapy alone is not recommended for managing tinea capitis (Strength of recommendation B; level of evidence 2++). 1 Oral antifungal therapy is required to achieve clinical and mycological cure, prevent scarring, and eradicate the organism. 1
Role of Antifungal Shampoos
Antifungal shampoos serve a specific adjunctive purpose: reducing transmission of spores to family members and close contacts. 1
Recommended Shampoo Options:
Three shampoos have demonstrated efficacy for reducing spore transmission:
- Ketoconazole 2% (Strength of recommendation C; level of evidence 2+) 1
- Selenium sulfide 1% (Strength of recommendation C; level of evidence 2+) 1
- Povidone-iodine (Strength of recommendation C; level of evidence 2+) 1
Practical Application:
- Frequency: Typically twice weekly during oral antifungal treatment 2
- Duration: Continue throughout the course of oral therapy (usually 6-8 weeks for griseofulvin or 2-4 weeks for terbinafine)
- Purpose: Removes adherent scales and hastens eradication of viable spores from the scalp 3
Evidence on Selenium Sulfide Concentrations
Research demonstrates that 1% selenium sulfide (over-the-counter) is equally effective as 2.5% selenium sulfide (prescription) for sporicidal activity. 2 A randomized trial showed no difference between the two concentrations in time required to eliminate shedding of viable spores, making the less expensive 1% formulation a practical choice. 2
Evidence on Ketoconazole Monotherapy
While one study showed that ketoconazole 2% shampoo alone achieved complete cure in 33% of children with Trichophyton tonsurans after 8 weeks of daily use 4, this approach is not recommended based on current guidelines. The British Association of Dermatologists explicitly states that although a small percentage may clear with topical agents alone, this is insufficient for standard practice. 1
Essential Treatment Algorithm:
Always prescribe oral antifungal therapy (griseofulvin 20-25 mg/kg/day for 6-8 weeks OR terbinafine based on weight for 2-4 weeks) 1
Add adjunctive shampoo (ketoconazole 2% or selenium sulfide 1%) twice weekly 1, 5
Screen and treat family members, especially for T. tonsurans infections where >50% of household contacts may be affected 1
Follow until mycological cure, not just clinical improvement—repeat cultures every 2 weeks until negative 1, 5
Common Pitfall to Avoid:
Never treat tinea capitis with shampoo alone. This leads to treatment failure, continued transmission, potential scarring alopecia, and prolonged infection. The fungus invades the hair shaft where topical agents cannot penetrate effectively. 1
