Desirable QTc Range During Tikosyn Loading
The baseline QTc must be ≤440 msec (≤500 msec in patients with ventricular conduction abnormalities) to initiate Tikosyn, and during loading, the QTc must remain <500 msec (or <550 msec with ventricular conduction abnormalities) to continue therapy. 111
Pre-Loading Requirements
Before administering the first dose of Tikosyn, the QTc must be determined using an average of 5-10 beats. If the baseline QTc exceeds 440 msec (or 500 msec in patients with ventricular conduction abnormalities), Tikosyn is absolutely contraindicated and should not be initiated. 111
During Loading Monitoring Protocol
Critical QTc Thresholds During Loading:
At 2-3 hours after each dose (doses 1-5), measure the QTc and apply these decision rules:
If QTc increases by >15% from baseline OR reaches >500 msec (>550 msec with conduction abnormalities): Reduce the dose according to the down-titration algorithm 11
If QTc exceeds 500 msec (550 msec with conduction abnormalities) at any time after the second dose: Discontinue Tikosyn immediately 11
Dose Adjustment Algorithm for QTc Prolongation:
The FDA label provides explicit guidance 11:
- If starting dose was 500 mcg BID → reduce to 250 mcg BID
- If starting dose was 250 mcg BID → reduce to 125 mcg BID
- If starting dose was 125 mcg BID → reduce to 125 mcg once daily
Duration of Monitoring
Continuous ECG monitoring is mandatory for a minimum of 3 days, or for at least 12 hours after electrical or pharmacological cardioversion to normal sinus rhythm, whichever is greater. 11 The 2023 ACC/AHA/ACCP/HRS guidelines confirm this requirement for continuous electrocardiographic monitoring during hospitalization for dofetilide initiation. 23
Common Pitfalls to Avoid
Baseline QTc >440 msec is the most frequent protocol deviation (occurring in 25% of patients in one real-world study), yet this represents an absolute contraindication. 4 Despite this being a clear contraindication, clinicians frequently deviate from this requirement, which may increase adverse event risk.
Use QT interval (not QTc) if heart rate is <60 bpm. Patients with heart rates <50 bpm have not been studied and represent uncharted territory. 111
Maintenance Phase
After successful loading, QTc should be reassessed every 3-6 months (more frequently if taking other QT-prolonging drugs or with changing renal function). If maintenance QTc exceeds 500 msec (550 msec with conduction abnormalities), discontinue Tikosyn and monitor until QTc returns to baseline. 11 23
The evidence demonstrates that ventricular tachyarrhythmias occur in approximately 1-4% of patients during loading, with torsades de pointes risk being dose-related. 456 Strict adherence to these QTc thresholds is essential for minimizing life-threatening proarrhythmic events.
