Doxorubicin-Ifosfamide-Mesna Protocol for High-Grade Soft Tissue Sarcoma
For adults with high-grade soft tissue sarcoma, administer doxorubicin 60 mg/m² IV on day 1, ifosfamide 2.5 g/m² IV over 6 hours on days 1-3, and mesna 2.5 g/m² IV over 6 hours on days 1-3, repeated every 21 days. This represents the standard anthracycline-based combination regimen endorsed by major guidelines for advanced disease 1, 2.
Dosing Schedule
The typical 3-day protocol follows this structure:
Day 1:
- Doxorubicin 60 mg/m² IV over 2 hours
- Ifosfamide 2.5 g/m² IV over 6 hours
- Mesna 2.5 g/m² IV over 6 hours (concurrent with ifosfamide)
Days 2-3:
- Ifosfamide 2.5 g/m² IV over 6 hours daily
- Mesna 2.5 g/m² IV over 6 hours daily (concurrent with ifosfamide)
Repeat cycle every 21 days 3, 4.
Mesna Administration Details
According to FDA labeling, mesna must be given to prevent hemorrhagic cystitis from ifosfamide 5. The standard approach uses:
- Mesna dose = 20% of ifosfamide dose at time of ifosfamide administration, then again at 4 and 8 hours after each ifosfamide dose (total daily mesna = 60% of ifosfamide dose)
- Alternative continuous infusion: Mesna can be given as continuous infusion concurrent with ifosfamide at equal weight-to-weight dosing (100% of ifosfamide dose)
The continuous infusion method is preferred in most modern protocols as it simplifies administration and may reduce CNS and renal toxicity 6, 7.
Clinical Context and Evidence
First-line therapy: Both NCCN and ESMO-EURACAN guidelines support anthracycline-based combination regimens (doxorubicin with ifosfamide) as standard first-line treatment for advanced, unresectable, or metastatic soft tissue sarcoma 1, 2. This combination is more effective than single-agent therapy in appropriate candidates.
Response rates: The doxorubicin-ifosfamide combination achieves objective response rates of 40-50% in treatment-naïve patients 3, 4. A 2021 retrospective analysis showed this regimen independently improved distant metastasis-free survival, disease-specific survival, and overall survival in high-grade tumors >5 cm 4.
Histology considerations: This protocol is appropriate for most high-grade soft tissue sarcoma subtypes including undifferentiated pleomorphic sarcoma, leiomyosarcoma, synovial sarcoma, and dedifferentiated liposarcoma. However, recognize that certain histologies have preferred alternatives (e.g., gemcitabine-docetaxel for leiomyosarcoma, trabectedin for liposarcoma) 2.
Toxicity Management
Hematologic toxicity is dose-limiting:
- Grade 3-4 neutropenia occurs in 80-95% of patients 8, 9
- Mandatory: Administer G-CSF (granulocyte colony-stimulating factor) starting day 4-5 and prophylactic antibiotics to prevent febrile neutropenia 8, 7
- Grade 3-4 thrombocytopenia and anemia occur in 20-50% 8
Monitor for hemorrhagic cystitis:
- Maintain adequate hydration and urinary output throughout treatment 5
- Check urine for hematuria before each cycle
- If severe hematuria develops despite mesna, reduce or discontinue ifosfamide 5
Dose modifications: Most patients require dose reduction to 60-80% of planned doses due to cumulative toxicity 10. This is acceptable and does not necessarily compromise efficacy.
Duration of Treatment
- Advanced/metastatic disease: Continue until disease progression, unacceptable toxicity, or cumulative doxorubicin dose reaches 450-550 mg/m² (cardiac toxicity threshold)
- Neoadjuvant setting: Typically 3 cycles before surgery, followed by 3 cycles adjuvant 7
- Typical duration: 4-6 cycles for most patients 10, 7
Important Caveats
Compatibility warning: Do not mix mesna with epirubicin, cyclophosphamide, cisplatin, carboplatin, or nitrogen mustard in the same bag 5. Ifosfamide and mesna may be mixed if final ifosfamide concentration does not exceed 50 mg/mL 5.
Patient selection: This intensive regimen is appropriate for fit patients with good performance status. For elderly or frail patients, consider single-agent doxorubicin or alternative regimens 2.
Cardiac monitoring: Assess baseline cardiac function before starting doxorubicin and monitor cumulative dose to prevent cardiomyopathy.
