Tinzaparin Bridging Dosing
For bridging anticoagulation, tinzaparin should be dosed at 175 IU/kg subcutaneously once daily, initiated 3-4 days before the procedure when warfarin is stopped, and resumed postoperatively for approximately 6 days until therapeutic INR is re-established. 1, 2
Weight-Based Dosing
The standard therapeutic dose of tinzaparin for bridging is 175 IU/kg subcutaneously once daily 1, 2, 1. This is the same dose used for treatment of established VTE and should be applied to bridging scenarios where full anticoagulation is required to prevent arterial thromboembolism.
- Administered as a single daily subcutaneous injection
- No dose capping for obesity—actual body weight should be used 3
- Recent evidence confirms weight-adjusted dosing achieves appropriate anti-Xa levels across weight ranges without accumulation 3
Renal Adjustment: Critical Considerations
Tinzaparin has unique renal handling compared to other LMWHs and requires careful consideration in renal insufficiency:
Moderate Renal Impairment (CrCl 30-60 mL/min)
- Do NOT empirically reduce the dose 4
- Studies show preemptive 25% dose reduction leads to inadequate anti-Xa levels in 92.3% of patients 4
- Full therapeutic dosing (175 IU/kg) can be used with monitoring 4
- Consider anti-Xa monitoring if available (target 0.5-1.5 IU/mL, measured 4-6 hours post-dose)
Severe Renal Impairment (CrCl <30 mL/min)
- Avoid tinzaparin in patients ≥70 years old with severe renal insufficiency 5, 6
- This recommendation stems from a randomized trial showing 11.2% vs 6.3% mortality (p=0.049) with tinzaparin vs UFH in elderly patients with CrCl <60 mL/min 5, 6
- If tinzaparin must be used in younger patients with severe renal impairment, anti-Xa monitoring is mandatory 6
- Alternative: Consider unfractionated heparin for bridging in this population
Hemodialysis Patients
- Weight-based dosing at 75 IU/kg has been studied and appears safe 7, 8, 9
- Anti-Xa activity returns to baseline within 24 hours without accumulation 7
- Fixed dosing protocols have shown efficacy without bleeding complications 8, 9
Timing of Initiation and Discontinuation
Pre-Procedure Protocol
- Stop warfarin 5 days before the procedure 10
- Start tinzaparin 3-4 days before surgery (when INR begins to fall below therapeutic range) 10
- Last preoperative dose: 24 hours before surgery for most procedures
- Hold tinzaparin for 48 hours before high-bleeding-risk procedures (neuraxial anesthesia, neurosurgery)
Post-Procedure Protocol
- Resume tinzaparin 12-24 hours postoperatively for low-bleeding-risk procedures 10
- Resume tinzaparin 48-72 hours postoperatively for high-bleeding-risk procedures
- Restart warfarin the evening of or day after the procedure with a 50% boost dose for 2 days, then maintenance dosing 10
- Continue tinzaparin for minimum 5-7 days and until INR is therapeutic (≥2.0) for 2 consecutive days 1
- Total bridging duration typically 6 days postoperatively 10
Common Pitfalls to Avoid
Do not empirically reduce tinzaparin dose in moderate renal impairment—this leads to subtherapeutic anticoagulation 4
Do not use tinzaparin in elderly patients (≥70 years) with CrCl <60 mL/min—use UFH instead 5, 6
Do not cap the dose in obese patients—use actual body weight for all calculations 3
Do not confuse bridging doses with prophylactic doses—bridging requires therapeutic dosing (175 IU/kg), not prophylactic dosing (4500 IU or 75 IU/kg) 11, 6
Do not restart tinzaparin too early postoperatively in high-bleeding-risk procedures—wait 48-72 hours for neurosurgery or spinal procedures
Do not stop tinzaparin before achieving therapeutic INR—overlap must continue until INR ≥2.0 for 2 consecutive days 1
