Age Ranges for Hepatitis B Vaccines
Engerix-B and Recombivax HB are approved for all ages from birth onward, while Heplisav-B is approved only for adults aged ≥18 years. 1, 2, 3
Specific Age Approvals by Vaccine
Engerix-B (Hepatitis B Vaccine)
- Approved for: All ages from birth onward 1, 3
- Dosing varies by age:
- Infants (<1 year): 10 μg (0.5 mL)
- Children (1-10 years): 10 μg (0.5 mL)
- Adolescents (11-19 years): 10 μg (0.5 mL)
- Adults (≥20 years): 20 μg (1.0 mL)
Recombivax HB (Hepatitis B Vaccine)
- Approved for: All ages from birth onward 1
- Dosing varies by age:
- Infants (<1 year): 5 μg (0.5 mL)
- Children (1-10 years): 5 μg (0.5 mL)
- Adolescents (11-15 years): 10 μg (1.0 mL) adult formulation on 2-dose schedule
- Adolescents (11-19 years): 5 μg (0.5 mL) on standard 3-dose schedule
- Adults (≥20 years): 10 μg (1.0 mL)
Heplisav-B (Hepatitis B Vaccine)
- Approved for: Adults aged ≥18 years ONLY 1, 2
- NOT approved for infants, children, or adolescents <18 years 1
- The guidelines explicitly state: "Heplisav-B should not be used for vaccination of infants, children, or adolescents because the safety and effectiveness of Heplisav-B has not been established in persons aged <18 years and is not approved for use in these populations" 1
- Dosing: 20 μg (0.5 mL) on a 2-dose schedule at 0 and 1 month
Important Clinical Considerations
For pregnant women: Use Engerix-B, Recombivax HB, or Twinrix (combination vaccine). Do not use Heplisav-B or PreHevbrio during pregnancy due to insufficient safety data. 2
For hemodialysis patients: Heplisav-B and PreHevbrio have not been established as safe or effective in adults on hemodialysis. Use Engerix-B or Recombivax HB with higher doses (40 μg) for this population. 2
Key pitfall to avoid: Do not inadvertently use Heplisav-B in anyone under 18 years of age, as it lacks FDA approval and safety data for pediatric populations. The 2021 CDC guidelines make this restriction explicit to prevent off-label use in younger age groups. 1
