Aztreonam Dosage for Acute Pyelonephritis
Aztreonam is not a first-line agent for acute pyelonephritis and should only be used when other recommended antibiotics cannot be administered due to allergies or resistance patterns. The standard dosing for adults with normal renal function is 1-2 g IV every 8 hours, with dose reductions required for renal impairment 1.
Why Aztreonam is Not Preferred
The IDSA/ESMID guidelines for acute pyelonephritis do not list aztreonam as a recommended agent 2. The guidelines clearly state that hospitalized patients with pyelonephritis should receive fluoroquinolones, aminoglycosides (with or without ampicillin), extended-spectrum cephalosporins or penicillins (with or without aminoglycosides), or carbapenems 2. Aztreonam, as a monobactam β-lactam, falls into the broader category of β-lactams which are noted to be "less effective than other available agents for treatment of pyelonephritis" 2.
Standard Dosing (Normal Renal Function)
When aztreonam must be used for pyelonephritis:
- For moderately severe systemic infections: 1-2 g IV every 8-12 hours 1
- For severe or life-threatening infections: 2 g IV every 6-8 hours 1
The FDA label specifies that for severe systemic infections, the intravenous route is recommended, and therapy should continue for at least 48 hours after the patient becomes asymptomatic or evidence of bacterial eradication is obtained 1. Duration should be 10-14 days for β-lactam agents treating pyelonephritis 2.
Renal Dose Adjustments
Aztreonam requires significant dose reduction in renal impairment because it is primarily renally eliminated (60-70% recovered in urine by 8 hours) 1:
Creatinine Clearance 10-30 mL/min/1.73 m²
- Give normal loading dose (1-2 g)
- Reduce maintenance dose to 50% of usual dose at the same interval 1
- Example: If usual dose is 2 g q8h, give 1 g q8h
Creatinine Clearance <10 mL/min/1.73 m²
- Give normal loading dose (500 mg, 1 g, or 2 g)
- Maintenance dose: 25% of usual initial dose at usual intervals (q6-12h) 1
- After each hemodialysis session, give an additional 1/8 of the initial dose 1
Simplified Hemodialysis Dosing
Recent evidence supports simpler dosing: 1-2 g administered once daily after hemodialysis achieves >90% probability of target attainment for MICs up to 4-8 mg/L respectively 3. This is more practical than the complex FDA-recommended regimen, though thrice-weekly dosing should be avoided except in nonsevere infections with very low MICs (≤0.5 mg/L) 3.
Calculating Creatinine Clearance
When only serum creatinine is available, use the Cockcroft-Gault equation 1:
Males: CrCl = [weight (kg) × (140 - age)] / [72 × serum creatinine (mg/dL)]
Females: 0.85 × above value
Important Clinical Caveats
Elderly patients warrant special attention because serum creatinine may not accurately reflect renal function 1. Always calculate creatinine clearance rather than relying on serum creatinine alone in this population.
The metabolite SQ 26,992 accumulates in renal impairment (half-life 26 hours), particularly in elderly patients with creatinine clearance <50 mL/min 4. While this metabolite is microbiologically inactive and no adverse effects have been documented, monitoring is prudent during prolonged therapy 4.
Pharmacokinetic studies demonstrate that aztreonam clearance correlates closely with creatinine clearance (r=0.97) 5, and the elimination half-life increases from 2 hours in normal subjects to 6 hours in anephric patients 5. This strong correlation validates the dose adjustment recommendations.
Practical Algorithm
- Confirm aztreonam is necessary (allergy to preferred agents or documented resistance)
- Calculate creatinine clearance using Cockcroft-Gault equation
- Select loading dose based on infection severity (1-2 g)
- Adjust maintenance dosing:
- CrCl >30: Standard dosing (1-2 g q8-12h)
- CrCl 10-30: 50% dose reduction
- CrCl <10: 75% dose reduction + post-HD supplementation
- Plan for 10-14 days total duration 2
- Consider initial long-acting parenteral agent (ceftriaxone 1 g or aminoglycoside) if using oral step-down, as recommended for all β-lactams in pyelonephritis 2
