Recommended Adult Dosage of Letrozole
The standard recommended dose of letrozole is 2.5 mg taken orally once daily, without regard to meals 1.
Dosing by Clinical Indication
For Adjuvant Treatment of Early Breast Cancer
- Dose: 2.5 mg once daily
- Duration: The optimal duration is not definitively established, but median treatment duration in major trials was 5 years 1
- Treatment should be discontinued at disease relapse 2
For Extended Adjuvant Treatment
- Dose: 2.5 mg once daily
- Duration: Up to 5 additional years after completing initial adjuvant therapy
- In the pivotal MA.17R trial, letrozole 2.5 mg daily for 5 years was compared to placebo in women who had already completed 4.5-6 years of aromatase inhibitor therapy 2
- The ASCO 2019 guidelines support this dosing based on evidence showing improved disease-free survival 2
- Both continuous and intermittent regimens have been studied: continuous 2.5 mg/day versus intermittent 2.5 mg/day for 9 months per year in years 1-4, then continuous in year 5 2
For Advanced/Metastatic Breast Cancer
- Dose: 2.5 mg once daily
- Duration: Continue until tumor progression is evident 1
- This dose was superior to both 0.5 mg letrozole and megestrol acetate in achieving objective response rates (24% vs 13% vs 16%, respectively) 3
Special Populations
Hepatic Impairment
- Mild to moderate impairment: No dose adjustment needed
- Cirrhosis or severe hepatic dysfunction: Reduce dose by 50% to 2.5 mg every other day 1
Renal Impairment
- No dose adjustment required if creatinine clearance ≥10 mL/min 1
Key Clinical Considerations
The 2.5 mg daily dose is consistently supported across all major guidelines and FDA labeling. The MA.17R trial demonstrated that this dose, when extended beyond 5 years of initial therapy, significantly improved 5-year disease-free survival (95% vs 91% with placebo, HR 0.79) 2. The NSABP B-42 trial similarly showed benefit with 2.5 mg daily (HR 0.85 for disease-free survival) 2.
Common Pitfalls to Avoid
- Do not use lower doses (0.5 mg): This was shown to be significantly less effective than 2.5 mg in head-to-head trials 3
- Monitor bone health: Extended letrozole therapy increases risk of osteoporosis and fractures (NNH = 72 for bone fractures) 2
- Consider cardiovascular risk: Meta-analysis showed increased odds of cardiovascular events (OR 1.18, NNH = 122) with extended AI therapy 2
The evidence strongly supports 2.5 mg once daily as the optimal dose across all breast cancer treatment settings, with duration tailored to the specific clinical indication (adjuvant vs extended adjuvant vs metastatic disease).
