Zenpep Dosing and Administration for Adults
Start adult patients on Zenpep at 40,000 USP units of lipase with each meal (or 500 lipase units/kg/meal), taken during the meal, with half that dose (20,000 units) for snacks. 1, 2
Starting Dose Algorithm
The initial dosing is straightforward and based on the 2023 AGA guidelines combined with FDA labeling:
- Main meals: 40,000 USP units of lipase (equivalent to 500 units/kg for an 80 kg patient)
- Snacks: 20,000 USP units of lipase (half the meal dose)
- Frequency: Three meals plus two to three snacks daily
This translates practically to two 20,000-unit Zenpep capsules per meal and one 20,000-unit capsule per snack for most adults. 1, 2
Critical Administration Instructions
Timing is essential—Zenpep must be taken during the meal, not before or after, because pancreatic enzyme replacement therapy treats the meal, not the pancreas, and requires mixing with food for optimal digestion. 1
For patients who can swallow capsules:
- Swallow capsules whole during the meal
- Drink sufficient water or juice to ensure complete swallowing 2
For patients unable to swallow capsules:
- Open capsules and sprinkle entire contents on acidic soft food (pH ≤4.5) such as applesauce, bananas, or pears
- Consume immediately—do not store
- Never crush or chew the capsule contents (destroys enteric coating) 2
Titration Schedule
If steatorrhea or malabsorption symptoms persist after several days, increase the dose systematically:
- First escalation: Double the dose to 80,000 units/meal and 40,000 units/snack
- Maximum safe dose: 2,500 lipase units/kg/meal OR 10,000 lipase units/kg/day OR 4,000 lipase units/g fat ingested/day—whichever is lowest 1, 2
For an 80 kg patient, this maximum translates to approximately 200,000 units per meal or 800,000 units daily total. Do not exceed these limits without further investigation for fibrosing colonopathy risk. 2
The dose should be adjusted based on:
- Meal size and fat content (higher fat meals require more enzymes)
- Persistence of steatorrhea
- Weight gain or loss
- Nutritional markers 1
Important Considerations
Zenpep is enteric-coated with pH-sensitive beads, meaning it does not require co-administration with acid-reducing agents (unlike non-enteric-coated Viokace). However, many patients are already on PPIs or H2-blockers, which may improve PERT efficacy. 1
All pancreatic enzyme products are equipotent at similar lipase doses—there is generally no therapeutic reason to switch between brands, though insurance coverage may necessitate changes. When switching from another product to Zenpep, monitor for clinical symptoms and retitrate as needed. 1, 2
Common Pitfalls to Avoid
- Taking enzymes before or after meals rather than during—this dramatically reduces efficacy
- Crushing or chewing capsules—destroys the enteric coating and inactivates enzymes in stomach acid
- Using over-the-counter enzyme supplements—these are unregulated dietary supplements with unknown efficacy and safety 1
- Underdosing—40,000 units/meal is a starting point; many patients require higher doses for adequate symptom control 3
- Forgetting snack doses—snacks require enzyme coverage too, at half the meal dose
Monitoring Treatment Success
Measure response to therapy through:
- Reduction in steatorrhea and GI symptoms (bloating, flatulence, diarrhea)
- Weight gain and improved muscle mass
- Improved fat-soluble vitamin levels (A, D, E, K)
- Nutritional status markers (BMI, albumin) 1
Stable patients should be reassessed at least annually with fat-soluble vitamin levels, nutritional assessment, and DEXA scan every 1-2 years for bone health monitoring. 1
If symptoms persist despite adequate dosing, consider adding a PPI or H2-blocker to optimize duodenal pH for enzyme release, or investigate alternative causes of malabsorption (celiac disease, small intestinal bacterial overgrowth, bile acid diarrhea). 1
