From the Guidelines

The dosing range for citalopram is 20 to 40 mg per day for outpatients, as indicated in the American Family Physician journal 1. When determining the appropriate dose, it's essential to consider individual patient factors, such as renal or hepatic disease, as the study suggests considering a decrease in dose for patients with these conditions 1. Key points to consider when prescribing citalopram include:

  • Initial dose: 20 mg once daily for adults with depression
  • Maximum dose: 40 mg daily if needed after at least one week
  • Dose adjustment for older adults or those with liver impairment: starting at 10 mg daily with a maximum of 20 mg
  • Importance of gradual dose changes to avoid withdrawal symptoms
  • Potential for QT prolongation at higher doses, emphasizing the need for careful dose selection and monitoring 1. It is crucial to consult a healthcare provider to determine the specific dosing range based on individual needs, medical conditions, and response to treatment.

From the FDA Drug Label

DOSAGE AND ADMINISTRATION Citalopram tablets (citalopram HBr) should be administered once daily, in the morning or evening, with or without food. Initial Treatment Citalopram tablets should be administered at an initial dose of 20 mg once daily, with an increase to a maximum dose of 40 mg/day at an interval of no less than one week Doses above 40 mg/day are not recommended due to the risk of QT prolongation. 20 mg/day is the maximum recommended dose for patients who are greater than 60 years of age, patients with hepatic impairment, and for CYP2C19 poor metabolizers or those patients taking cimetidine or another CYP2C19 inhibitor.

The dosing range for citalopram is:

  • Initial dose: 20 mg once daily
  • Maximum dose: 40 mg/day
  • Special populations: 20 mg/day for patients greater than 60 years of age, patients with hepatic impairment, and for CYP2C19 poor metabolizers or those patients taking cimetidine or another CYP2C19 inhibitor 2

From the Research

Dosing Range for Citalopram

The dosing range for citalopram can vary depending on the individual and the specific condition being treated.

  • The minimal effective dose of citalopram was shown to be 20 mg 3.
  • Statistically significant differences between citalopram and placebo were found at both the 20 and 40 mg dose levels 3.
  • Analysis of patient subgroups revealed a tendency for those patients suffering from severe or recurrent depression to achieve better results with a higher dosage (40 mg), while patients experiencing their first period of depression or with less severe depression responded well to the minimally effective dose of 20 mg 3.
  • In the STAR*D study, patients were analyzed according to dosage -20 or 40 mg per day of citalopram, and the results indicated that citalopram amplifies the influence of the living conditions on mood in a dose-dependent manner 4.
