Leucovorin Dosing Prior to 5-Fluorouracil Chemotherapy
The recommended leucovorin dose is 400 mg/m² IV over 2 hours on day 1, followed by 5-FU bolus and continuous infusion, repeated every 2 weeks (mFOLFOX6/FOLFIRI regimens). Alternatively, 200 mg/m² levoleucovorin is equivalent and can be substituted. 1
Standard Dosing Regimens
Biweekly Infusional Regimens (Preferred)
mFOLFOX6 and FOLFIRI (Most Common):
- Leucovorin: 400 mg/m² IV over 2 hours, day 1
- Followed by 5-FU 400 mg/m² IV bolus, then 1200 mg/m²/day × 2 days continuous infusion
- Repeat every 2 weeks 1, 2
FOLFOX4 (Alternative):
- Leucovorin: 200 mg/m² IV over 2 hours, days 1 and 2
- Followed by 5-FU 400 mg/m² IV bolus, then 600 mg/m² over 22 hours on days 1 and 2
- Repeat every 2 weeks 1
Weekly Regimens
Roswell-Park Regimen:
- Leucovorin: 500 mg/m² IV over 2 hours
- 5-FU: 500 mg/m² IV bolus 1 hour after start of leucovorin
- Given days 1,8,15,22,29,36; repeat every 8 weeks 1
Low-Dose Weekly:
- Leucovorin: 20 mg/m² IV over 2 hours
- 5-FU: 500 mg/m² IV bolus 1 hour after leucovorin
- Repeat weekly 1
Daily × 5 Regimens (FDA-Approved)
High-Dose Option:
- Leucovorin: 200 mg/m² IV over minimum 3 minutes
- 5-FU: 370 mg/m² IV bolus
- Daily × 5 days, repeat every 4 weeks 3
Low-Dose Option:
- Leucovorin: 20 mg/m² IV
- 5-FU: 425 mg/m² IV bolus
- Daily × 5 days, repeat every 4-5 weeks 3
Levoleucovorin Equivalency
Critical conversion: 200 mg/m² levoleucovorin = 400 mg/m² standard leucovorin 1, 2
This is important because levoleucovorin is the active L-isomer with superior transport properties (8-fold higher affinity for RFC, 3.5-fold higher for PCFT) compared to the D-isomer 4. The racemic mixture contains 50% inactive D-leucovorin, hence the 2:1 dosing ratio.
Renal Impairment Adjustments
Leucovorin dosing does NOT require adjustment for renal dysfunction 3. However:
- Capecitabine requires dose modification with diminished creatinine clearance 1
- A phase I study demonstrated that patients with creatinine 1.5-3.0 mg/dL safely tolerated leucovorin 500 mg/m² with 5-FU 2600 mg/m² as 24-hour infusion without dose adjustment 5
- 5-FU clearance showed no relationship to serum creatinine levels 5
The key distinction: while 5-FU itself may not require renal dose adjustment, oral fluoropyrimidines (capecitabine) do require modification based on creatinine clearance.
Practical Considerations
Leucovorin Shortage Management
During shortages, NCCN recommends 6:
- Switch to levoleucovorin (200 mg/m² equivalent to 400 mg/m²)
- Use lower leucovorin doses (20-25 mg/m² vs. 200-500 mg/m²) - likely equally efficacious based on multiple studies showing no survival difference 6
- Omit leucovorin entirely if unavailable, with consideration for modest 5-FU dose increase (~10%) if no grade ≥2 toxicity occurs
Dose Intensity Optimization
Lower leucovorin doses may improve tolerability without compromising efficacy:
- Modified FOLFOX with leucovorin 20 mg/m² (vs. standard 200-400 mg/m²) showed 49% response rate with reduced toxicity 7
- This suggests the standard high-dose leucovorin may be unnecessarily toxic
Administration Timing
Leucovorin must be given BEFORE 5-FU to allow:
- Adequate time for cellular uptake and conversion to 5,10-methylenetetrahydrofolate
- Formation of the stable ternary complex with thymidylate synthase and 5-FU metabolites
- Typical timing: 5-FU bolus given 1 hour after start of leucovorin infusion (Roswell-Park) or immediately following leucovorin completion (FOLFOX/FOLFIRI)
Common Pitfalls
- Do NOT administer leucovorin intrathecally - potentially fatal 3
- Administer 5-FU and leucovorin separately to avoid precipitate formation 3
- Do not confuse leucovorin rescue dosing (for methotrexate toxicity) with 5-FU modulation dosing - these are entirely different indications with different doses
- Leucovorin doses are NOT adjusted for toxicity - only 5-FU doses are modified 3
