From the Guidelines

Acyclovir is the correct medication name, not wcyclovir, and it is an antiviral medication used to treat infections caused by herpes viruses, including herpes simplex (cold sores, genital herpes), herpes zoster (shingles), and varicella (chickenpox). The treatment for genital herpes typically involves taking acyclovir 400 mg three times daily for 7-10 days for initial outbreaks, and 400 mg three times daily for 5 days for recurrent episodes 1. Some key points to consider when treating genital herpes with acyclovir include:

  • Daily suppressive therapy can reduce the frequency of genital herpes recurrences by greater than or equal to 75% among patients who have frequent recurrences (i.e., six or more recurrences per year) 1
  • Safety and efficacy have been documented among patients receiving daily therapy with acyclovir for as long as 6 years, and with valacyclovir and famciclovir for 1 year 1
  • If lesions persist in a patient receiving acyclovir treatment, resistance of the HSV strain to acyclovir should be suspected, and alternate therapy such as foscarnet or topical cidofovir gel may be effective 1 It's also important to note that acyclovir works by inhibiting viral DNA synthesis, preventing the virus from replicating, and that patients with kidney problems may need dose adjustments 1. In cases of severe disease, IV acyclovir therapy may be required, and hospitalization should be considered for severe disease caused by proven or suspected acyclovir-resistant strains 1. Overall, the goal of treatment is to reduce the frequency and severity of symptoms, and to improve quality of life for patients with genital herpes.

From the FDA Drug Label

Acyclovir tablets is indicated for the acute treatment of herpes zoster (shingles). Acyclovir tablets is indicated for the treatment of initial episodes and the management of recurrent episodes of genital herpes. Acyclovir tablets is indicated for the treatment of chickenpox (varicella).

The treatment for wcyclovir (likely a typo, meant to be acyclovir) includes:

  • Herpes Zoster Infections: Acyclovir tablets for the acute treatment of herpes zoster (shingles) 2
  • Genital Herpes: Acyclovir tablets for the treatment of initial episodes and the management of recurrent episodes of genital herpes 2
  • Chickenpox: Acyclovir tablets for the treatment of chickenpox (varicella) 2

From the Research

Treatment for Herpes Simplex

  • The treatment for herpes simplex, including genital herpes and herpes labialis, typically involves the use of antiviral medications such as valacyclovir, acyclovir, and famciclovir 3, 4, 5.
  • Valacyclovir has been shown to be effective in the treatment of initial genital herpes outbreaks, episodic treatment of recurrent episodes, and daily suppressive therapy 3, 4.
  • Famciclovir is also a well-tolerated first-line option for the treatment of herpes zoster and the treatment and suppression of genital herpes 6.

Treatment Options

  • Oral antiviral agents, such as acyclovir, valacyclovir, and famciclovir, are superior to topical antiviral therapy for episodic treatment of herpes labialis 7.
  • Topical antiviral agents, such as 5% acyclovir cream/ointment and 1% penciclovir cream, can also be used for episodic treatment of herpes labialis, but are not effective in preventing recurrent episodes 7.
  • Chronic daily suppressive therapy with oral antiviral agents is the treatment of choice for patients with severe or frequent recurrences of herpes labialis 7.

Efficacy and Safety

  • Valacyclovir has been shown to be safe and effective in the treatment of patients with herpes simplex and herpes zoster, with an acceptable safety profile 3, 4.
  • Famciclovir has also been shown to be well-tolerated and effective in the treatment of herpes zoster and genital herpes 6.
  • The use of antiviral therapy has been shown to reduce the duration of pain and discomfort, hasten healing, and reduce viral shedding in patients with herpes labialis 7.
