From the Guidelines

Valtrex (valacyclovir) is recommended for suppressive therapy to prevent HSV recurrences, with a dosage of 500 mg twice daily in HIV-infected persons 1. When considering treatment options for herpes simplex virus (HSV) infections, it's essential to prioritize regimens that are most feasible for patient adherence.

  • For initial genital herpes outbreaks, the recommended dosage is 1 gram twice daily for 10 days.
  • For recurrent episodes, take 500 mg twice daily for 3 days, starting at the first sign of symptoms.
  • For suppressive therapy to prevent outbreaks, 500 mg to 1 gram once daily is typically prescribed, but 500 mg twice daily is recommended for HIV-infected persons 1. According to the most recent guidelines, several dosing strategies of anti-herpesvirus medications have been studied, and dosing strategies that are most feasible for patient adherence should be prioritized 1. It's crucial to note that while Valtrex can reduce symptoms and decrease viral shedding, it cannot cure HSV infection. For maximum effectiveness, treatment should begin as soon as possible after symptoms appear. Common side effects of Valtrex include headache, nausea, and dizziness, and it's essential to stay well-hydrated while taking Valtrex and to complete the full course of treatment even if symptoms improve.

1 INDICATIONS AND USAGE

1.1 Adult Patients Genital Herpes Initial Episode: VALTREX is indicated for treatment of the initial episode of genital herpes in immunocompetent adults. Recurrent Episodes: VALTREX is indicated for treatment of recurrent episodes of genital herpes in immunocompetent adults.

The efficacy of treatment with VALTREX when initiated more than 72 hours after the onset of signs and symptoms has not been established for initial episodes, and more than 24 hours after the onset of signs and symptoms has not been established for recurrent episodes.

Valtrex is indicated for the treatment of HSV (Herpes Simplex Virus) in adults, specifically for:

  • Initial episodes of genital herpes
  • Recurrent episodes of genital herpes It is essential to initiate treatment within 72 hours for initial episodes and 24 hours for recurrent episodes after the onset of signs and symptoms for optimal efficacy 2.

From the Research

Valtrex for HSV Treatment

  • Valtrex, also known as valaciclovir, is an antiviral medication used to treat infections caused by herpes simplex virus (HSV) and varicella zoster virus 3, 4.
  • It is an effective treatment for herpes zoster in immunocompetent adults, and has been shown to reduce the duration of zoster-associated pain 4.
  • Valtrex is also effective in treating genital herpes, reducing the duration of viral shedding, hastening lesion healing, and decreasing lesion-associated pain 3, 4.

Efficacy of Valtrex for HSV Suppression

  • Studies have demonstrated the efficacy of long-term therapy with valaciclovir for the suppression of genital HSV disease in otherwise healthy individuals with HSV infection 3.
  • Valaciclovir dosages of ≥ 500 mg daily are recommended for suppression of genital herpes recurrences in immunocompetent individuals 3.
  • Comparative studies have shown that valacyclovir may be more effective than famciclovir for suppression of genital herpes and associated shedding 5.

Safety and Tolerability of Valtrex

  • Valtrex is well tolerated at dosages used to suppress recurrent episodes of genital herpes (500 to 1000 mg/day) in immunocompetent and HIV seropositive individuals 3, 4.
  • However, a potentially fatal thrombotic microangiopathy (TMA)-like syndrome has been reported in some immunocompromised patients receiving high-dose prophylactic valaciclovir therapy 3.
  • Close monitoring for symptoms of TMA is indicated in all immunocompromised patients receiving high-dose valaciclovir 3.

Comparative Studies with Other Antivirals

  • Studies have compared the efficacy of valaciclovir with famciclovir and aciclovir for the treatment of genital herpes and herpes zoster 4, 5, 6.
  • While valacyclovir and famciclovir have been shown to be effective in limiting the virulence and spread of HSV-1, valacyclovir may have a better effect on reducing the latent viral DNA load 6.
