Differential Diagnosis
The patient's laboratory results show elevated glucose, carbon dioxide, and alkaline phosphatase levels, along with low calcium, protein, and albumin levels. Based on these findings, the following differential diagnoses can be considered:
- Single most likely diagnosis
- Diabetes mellitus: The elevated glucose level (212) is a strong indicator of diabetes mellitus. The presence of other abnormalities, such as low albumin and high alkaline phosphatase, can be seen in patients with uncontrolled diabetes.
- Other Likely diagnoses
- Malnutrition: The low levels of protein, albumin, and calcium suggest malnutrition, which can be related to various factors, including poor diet, malabsorption, or increased protein loss.
- Liver disease: Although the liver enzymes (AST and ALT) are within normal limits, the elevated alkaline phosphatase level can indicate liver disease, such as cholestasis or bone-related disorders.
- Hyperparathyroidism: The low calcium level, despite the presence of other abnormalities, can be seen in hyperparathyroidism, particularly if the parathyroid hormone (PTH) level is elevated.
- Do Not Miss (ddxs that may not be likely, but would be deadly if missed.)
- Multiple myeloma: Although rare, multiple myeloma can present with low albumin, high alkaline phosphatase, and hypercalcemia (not seen in this case). However, it is essential to consider this diagnosis due to its potential severity.
- Vitamin D deficiency: A deficiency in vitamin D can lead to low calcium levels and elevated alkaline phosphatase. Although not immediately life-threatening, vitamin D deficiency can have significant long-term consequences if left untreated.
- Rare diagnoses
- Osteomalacia: A rare condition characterized by softening of the bones due to vitamin D deficiency or other factors. The low calcium level and elevated alkaline phosphatase can be seen in osteomalacia.
- Celiac disease: An autoimmune disorder that can lead to malabsorption of nutrients, including calcium and protein. The low levels of these nutrients, along with the elevated alkaline phosphatase, can be seen in celiac disease.
