Differential Diagnosis for Prominence in the Upper Back/Shoulder to the Medial Cervical Spine
Single Most Likely Diagnosis
- Musculoskeletal strain or sprain: This is often the most common cause of prominence or discomfort in the upper back and shoulder area, usually resulting from overuse, poor posture, or acute injury. The medial cervical spine can be involved due to referred pain from the shoulder or upper back muscles.
Other Likely Diagnoses
- Cervical spondylosis: A condition characterized by wear and tear on the discs in the neck, which can lead to pain and stiffness in the neck and shoulder area.
- Thoracic outlet syndrome: Compression, injury, or irritation of the nerves and/or blood vessels (arteries and veins) in the thoracic outlet area, which can cause pain in the shoulders and neck.
- Scapulothoracic bursitis: Inflammation of the bursa between the scapula and the thoracic wall, leading to pain and swelling in the upper back.
- Tension headaches: Can refer pain to the neck and shoulder area, contributing to the sensation of prominence or discomfort.
Do Not Miss Diagnoses
- Cervical spine fracture or instability: Although less common, fractures or instability in the cervical spine can present with pain and prominence in the upper back and shoulder area, and are critical to identify due to the risk of neurological compromise.
- Spinal cord compression: Conditions such as herniated discs, spinal stenosis, or tumors can compress the spinal cord, leading to serious neurological symptoms if not promptly addressed.
- Pulmonary pathology (e.g., Pancoast tumor): A type of lung cancer located at the top of the lung that can invade nearby structures, including the spine, ribs, and nerves, causing shoulder and upper back pain.
Rare Diagnoses
- Scheuermann's kyphosis: A rare condition that affects the upper back, leading to an abnormal curvature of the spine, which can cause prominence in the upper back.
- Multiple myeloma or other bone cancers: Although rare, these conditions can cause bone lesions in the spine, leading to pain and deformity.
- Infectious processes (e.g., osteomyelitis, discitis): Infections of the bone or disc can present with pain and swelling in the affected area, including the upper back and shoulder region.
