Differential Diagnosis for Urinary Frequency, Hematuria, Urgency, and Delayed Stream

Single Most Likely Diagnosis

  • Urinary Tract Infection (UTI): This is the most likely diagnosis due to the combination of urinary frequency, urgency, and hematuria, which are classic symptoms of a UTI. The delayed stream could be related to discomfort or pain during urination.

Other Likely Diagnoses

  • Benign Prostatic Hyperplasia (BPH): Common in older men, BPH can cause urinary frequency, urgency, and a delayed stream due to obstruction of the urethra by an enlarged prostate. Hematuria can also occur, especially if there's an associated infection or bladder outlet obstruction.
  • Overactive Bladder: Characterized by urinary urgency, frequency, and sometimes incontinence, overactive bladder could explain most symptoms, though hematuria would require further investigation for another cause.
  • Kidney Stones: While more commonly associated with severe pain, kidney stones can cause urinary frequency, urgency, and hematuria. A delayed stream is less typical but could occur if a stone is obstructing the urethra.

Do Not Miss Diagnoses

  • Bladder Cancer: Although less common, bladder cancer is a critical diagnosis not to miss, as it can present with painless hematuria, and sometimes with urinary frequency and urgency. Delayed stream could be due to a tumor obstructing the urethra.
  • Prostate Cancer: Similar to BPH, prostate cancer can cause obstructive urinary symptoms like delayed stream, frequency, and urgency. Hematuria is less common but can occur, making it a diagnosis that should not be overlooked.
  • Septicemia or Sepsis from a UTI: In severe cases, a UTI can lead to sepsis, a life-threatening condition. Early recognition and treatment are crucial, especially in vulnerable populations like the elderly or those with compromised immune systems.

Rare Diagnoses

  • Interstitial Cystitis: A chronic condition causing bladder pressure, bladder pain, and sometimes pelvic pain, interstitial cystitis can lead to urinary frequency and urgency. Hematuria and delayed stream are less common but possible.
  • Tuberculosis of the Urinary Tract: Although rare in many parts of the world, urinary TB can cause a variety of symptoms including frequency, urgency, hematuria, and obstructive symptoms like delayed stream.
  • Schistosomiasis: In endemic areas, this parasitic infection can cause urinary symptoms including hematuria, frequency, and obstructive symptoms due to bladder and ureteral fibrosis.
