How to Administer Lansoprazole ODT

Place the lansoprazole orally disintegrating tablet (ODT) on the tongue without water and allow it to disintegrate rapidly, then swallow with saliva, or alternatively, disperse the tablet in a small volume of water (approximately 60 mL or 2 ounces) and swallow immediately. 1

Standard Oral Administration

Take lansoprazole ODT before meals for optimal efficacy 1. The tablet can be administered in two ways:

Direct Oral Method (Preferred)

  • Place the ODT directly on the tongue
  • Allow it to disintegrate rapidly without chewing or crushing
  • Swallow with saliva—no water required
  • This method is bioequivalent to capsule formulations 2

Water Dispersion Method

If the patient prefers or needs liquid administration:

  • Place the ODT in approximately 60 mL (about ¼ cup) of water
  • Use only apple juice, orange juice, or tomato juice as alternatives 1
  • Stir briefly
  • Swallow immediately
  • Rinse the glass with additional juice and swallow to ensure complete dose delivery 1

Administration via Feeding Tubes

For patients with nasogastric (NG) tubes ≥16 French:

  1. Place 40 mL of apple juice in a clean container
  2. Open the ODT and empty pellets into the apple juice
  3. Draw up the mixture using a catheter-tipped syringe
  4. Gently mix to prevent pellet settling
  5. Inject through the NG tube immediately
  6. Flush the tube with an additional 40 mL of apple juice 1

Important caveat: Research shows NG tube administration maintains 76% bioavailability and actually provides superior acid suppression compared to IV lansoprazole in critically ill patients 3. However, gastrostomy (G) tubes carry the greatest risk of clogging compared to NG or jejunal tubes 4.

Administration in Soft Foods (Alternative)

For patients who cannot swallow tablets but don't require tube feeding:

  • Open the capsule and sprinkle intact pellets on one tablespoon of:
    • Applesauce
    • ENSURE pudding
    • Cottage cheese
    • Yogurt
    • Strained pears
  • Swallow immediately without chewing 1

Critical Timing Considerations

  • Take at least 30 minutes before sucralfate if both medications are prescribed 1
  • Antacids may be used concomitantly 1
  • For GERD treatment, dosing 30-60 minutes before a meal provides optimal efficacy 5, 6

Common Pitfalls to Avoid

  • Never crush or chew the ODT or pellets—this destroys the enteric coating 1
  • Do not use foods or liquids other than those specified—they have not been clinically studied 1
  • Do not save prepared mixtures for later use—administer immediately 1
  • Do not double dose if a dose is missed; skip it if the next dose is due 1

Clinical Context

The ODT formulation demonstrates bioequivalence to capsules with comparable Cmax and AUC values 2. The Tmax occurs at approximately 1.8-2.0 hours with a half-life of 1.18 hours 2. Patient acceptance is high, with the vast majority finding the mouth feel acceptable and administration easy 7.
