Olanzapine Dosing
For adults with schizophrenia, start olanzapine at 5-10 mg once daily and target 10 mg/day within several days; for bipolar disorder, start at 10-15 mg once daily. 1
Schizophrenia Dosing
Adults
- Initial dose: 5-10 mg once daily (oral)
- Target dose: 10 mg/day, achieved within several days
- Titration: Increase/decrease in 5 mg increments at intervals of ≥1 week (steady state requires ~1 week)
- Therapeutic range: 10-15 mg/day demonstrated efficacy in trials
- Maximum: 20 mg/day (doses above this are not FDA-approved for routine use) 1
Important caveat: Doses above 10 mg/day were not more efficacious than 10 mg/day in clinical trials 1. Only increase beyond 10 mg/day after clinical assessment confirms inadequate response.
Adolescents (13-17 years)
- Initial dose: 2.5-5 mg once daily
- Target dose: 10 mg/day
- Titration: Increase/decrease in 2.5-5 mg increments
- Maximum studied: 20 mg/day (mean modal dose in trials: 12.5 mg/day) 1
Critical consideration: Adolescents have 27-34% higher drug exposure than adults and face substantially greater metabolic risks (weight gain, dyslipidemia). Consider other antipsychotics first in this population 2.
Bipolar I Disorder (Manic/Mixed Episodes)
Adults
- Monotherapy initial dose: 10-15 mg once daily
- With lithium/valproate: Start at 10 mg once daily 1
Adolescents
- Initial dose: 2.5-5 mg once daily
- Target dose: 10 mg/day 1
Special Populations Requiring Lower Starting Doses
Start at 5 mg/day in adults who are:
- Debilitated
- Predisposed to hypotensive reactions
- Nonsmoking females ≥65 years old
- Hepatically impaired
- Otherwise pharmacodynamically sensitive 1
For older/frail patients with delirium: Start at 2.5-5 mg once daily (usually at bedtime), reduce dose in hepatic impairment 3
Acute Agitation (Intramuscular)
- Standard dose: 10 mg IM
- Alternative doses: 5-7.5 mg IM when clinically warranted
- Maximum: 3 doses given 2-4 hours apart
- Critical safety step: Assess for orthostatic hypotension before each subsequent dose 1
Administration Details
- Timing: Once daily, without regard to meals
- Available formulations: Standard tablets (2.5,7.5,10,15,20 mg), orally disintegrating tablets (5,10,15,20 mg), IM injection (10 mg vial) 1
Treatment-Resistant Cases
While the FDA label caps dosing at 20 mg/day, emerging evidence suggests selected treatment-resistant patients may benefit from higher doses:
- Post-hoc analysis: Patients with higher baseline symptom severity showed better response at 40 mg/day versus 10 mg/day 4
- Case reports: Doses of 35-60 mg/day have been used successfully in treatment-resistant schizophrenia, including one adolescent case at 50 mg/day 5, 6
- Trade-off: Higher doses (40 mg/day) significantly increase weight gain (3.0 kg vs 1.9 kg at 10 mg/day) and prolactin elevation 4, 7
Clinical practice reality: In New York State psychiatric centers, >50% of patients received >20 mg/day by 2006, though this exceeds FDA approval 7. Consider high-dose olanzapine only after failed trials of standard dosing and when clozapine is not feasible.
Key Safety Warnings
- Avoid combining with benzodiazepines: Risk of oversedation and respiratory depression, with reported fatalities at high olanzapine doses 3
- Metabolic monitoring essential: Particularly critical in adolescents given their heightened risk for weight gain, dyslipidemia, and glucose abnormalities 2
- Orthostatic hypotension: Common, especially with IM dosing—assess before repeat doses 1
- Elderly with dementia: Black box warning for increased mortality—olanzapine is NOT approved for this population 1
Combination Therapy
When using olanzapine with fluoxetine for bipolar depression or treatment-resistant depression:
- Adults: Start 5 mg olanzapine + 20 mg fluoxetine once daily
- Adolescents (10-17 years): Start 2.5 mg olanzapine + 20 mg fluoxetine once daily
- Maximum studied: 18 mg olanzapine + 75 mg fluoxetine (adults); 12 mg olanzapine + 50 mg fluoxetine (adolescents) 1
