Atenolol Dosing and Management
Hypertension
For hypertension, start atenolol at 50 mg once daily, assess response at 1-2 weeks, and increase to 100 mg once daily if needed—doses beyond 100 mg provide no additional benefit. 1
Dosing Algorithm:
- Initial dose: 50 mg once daily (alone or with diuretic)
- Assessment timing: 1-2 weeks to see full effect
- Titration: Increase to 100 mg once daily if blood pressure target not achieved
- Maximum effective dose: 100 mg daily (higher doses unlikely to improve outcomes) 1
Important context: While the 2024 ESC guidelines recommend ACE inhibitors, ARBs, calcium channel blockers, and diuretics as first-line agents for hypertension 2, beta-blockers like atenolol are reserved for compelling indications (post-MI, heart failure with reduced ejection fraction, angina, or rate control) 2. Meta-analysis data suggest atenolol may be inferior to other antihypertensives for cardiovascular outcomes and mortality in uncomplicated hypertension 3, so consider alternative agents unless specific indications exist.
Chronic Stable Angina
For angina pectoris, initiate atenolol at 50 mg once daily, increase to 100 mg daily after one week if needed, and consider 200 mg once daily for optimal 24-hour control in refractory cases. 1
Dosing Algorithm:
- Initial dose: 50 mg once daily
- First titration (at 1 week): Increase to 100 mg once daily if suboptimal response
- Maximum dose: 200 mg once daily for patients requiring full 24-hour coverage 1
Key Clinical Points:
- Doses of 50-100 mg provide maximum early exercise tolerance improvement, but effects at 24 hours are attenuated to 50-75% 1
- The 200 mg once-daily dose achieves superior 24-hour control compared to lower doses 4, 5
- Once-daily dosing at 100-200 mg is as effective as twice-daily regimens 5
- Maximum antianginal benefit may not appear until 3 months of therapy despite stable drug levels 6
Supraventricular Arrhythmias
For SVT rate control, begin atenolol at 25-50 mg once daily and titrate to 100 mg once daily as tolerated, monitoring for bradycardia and hypotension. 7
Dosing per ACC/AHA/HRS Guidelines:
- Initial dose: 25-50 mg once daily
- Maintenance dose: Titrate based on heart rate response
- Maximum dose: 100 mg once daily 7
Critical Precautions:
- Contraindications: Greater than first-degree AV block, SA node dysfunction without pacemaker, decompensated heart failure, hypotension, reactive airway disease 7
- Avoid combining with other AV nodal blocking agents without careful monitoring 7
Special Populations
Elderly and Renal Impairment
Atenolol is renally excreted and requires dose reduction in elderly patients and those with renal dysfunction—start at 25 mg daily and adjust based on creatinine clearance. 1
Renal Dosing Algorithm:
- CrCl 15-35 mL/min: Maximum 50 mg daily
- CrCl <15 mL/min: Maximum 25 mg daily
- Hemodialysis: 25-50 mg after each dialysis session (hospital supervision required due to hypotension risk) 1
- Elderly without known renal disease: Consider starting at 25 mg daily 1
Critical caveat: Atenolol accumulation and half-life prolongation occur when creatinine clearance falls below 35 mL/min/1.73m² 1. Always assess renal function before initiating therapy.
Monitoring Parameters
Essential Monitoring:
- Blood pressure and heart rate: Before each dose adjustment and at steady state
- Renal function: Baseline and periodically, especially in elderly 1
- ECG monitoring: For arrhythmia indications 7
- Exercise tolerance and angina frequency: For angina patients (may take up to 3 months for full benefit) 6
Adverse Effects to Monitor:
- Bradycardia (dose-limiting)
- Hypotension
- Bronchospasm (avoid in reactive airway disease)
- Fatigue (occurred in 33% of patients on chronic therapy, dose-limiting in some) 6
Discontinuation
Never abruptly stop atenolol in angina patients—taper gradually while limiting physical activity to prevent rebound ischemia. 1
Timing of Administration
Administer atenolol at the same time each day at whatever time is most convenient for the patient to maximize adherence—diurnal timing does not affect cardiovascular outcomes 2.
