Tirzepatide Can Be Used Starting at Age 10 Years
Tirzepatide is FDA-approved for use in children and adolescents aged 10 to <18 years with type 2 diabetes, and for adults aged 18 years and older for both type 2 diabetes and obesity. 1
Age-Specific Indications
Pediatric Population (Ages 10-17 Years)
- Type 2 diabetes only: Tirzepatide is approved for youth aged 10 to <18 years with inadequately controlled type 2 diabetes 2
- The SURPASS-PEDS trial demonstrated significant efficacy in this age group, with mean HbA1c reduction of 2.23% and BMI reductions of 7.4-11.2% over 30 weeks 2
- Not currently approved for obesity alone in pediatric patients - this is a critical distinction from adult indications
Adult Population (Ages 18+ Years)
- Type 2 diabetes: FDA-approved in 2022 at doses of 5,10, and 15 mg weekly 1
- Obesity: FDA-approved in 2023 for adults with BMI ≥30 kg/m² or ≥27 kg/m² with weight-related comorbidities 1
- Obstructive sleep apnea: FDA-approved in December 2024 for moderate-to-severe OSA in adults with obesity 1
Clinical Considerations by Age
Youth (10-17 Years)
The pediatric approval is specifically for type 2 diabetes management when inadequately controlled with metformin and/or basal insulin 2. Real-world data from 74 youth patients (median age 18.4 years) showed significant reductions in HbA1c (8.0% to 7.2%) and BMI even in those with prior GLP-1 RA exposure 3.
Older Adults (≥65 Years)
Tirzepatide demonstrates efficacy across age groups, though weight loss effects may be more pronounced in younger patients 4. In patients ≥65 years, the medication significantly reduced both HbA1c and BMI over 6 months in both young-old (65-74 years) and old-old (≥75 years) groups 5. Critical safety consideration: In older adults using insulin, sulfonylureas, or glinides, proactive dose reduction of these agents is essential - 39.2% of older patients required such adjustments to prevent hypoglycemia 5.
Dosing Algorithm
Starting dose: 2.5 mg weekly for all ages (this is a lead-in dose, not a therapeutic dose) 1
Titration schedule: Increase monthly by 2.5 mg increments based on tolerability and glycemic/weight response 1
Maintenance doses:
Common Pitfalls
- Do not use for obesity alone in patients under 18 years - current approval is only for type 2 diabetes in this age group
- In older adults on secretagogues: Reduce doses of insulin, sulfonylureas, or glinides preemptively to avoid hypoglycemia 5
- Gastrointestinal side effects: Most common during dose escalation; counsel patients to eat smaller meals, avoid fatty foods, and stay hydrated 1
- Contraindications: Personal or family history of medullary thyroid cancer or multiple endocrine neoplasia type 2 6
