Jardiance Starting Dose for Type 2 Diabetes
Start with Jardiance 10 mg once daily in adults with type 2 diabetes. 1
Dosing Algorithm
Initial Dose
- 10 mg once daily is the recommended starting dose for all adults with type 2 diabetes 1
- Can be taken with or without food
- Administer in the morning
Dose Escalation
- If additional glycemic control is needed, increase to 25 mg once daily 1
- The 25 mg dose provides incrementally better HbA1c reduction (approximately 0.1-0.2% additional lowering) 2
- Both doses (10 mg and 25 mg) are FDA-approved and effective 1
Special Populations
Renal Function Considerations
- eGFR ≥30 mL/min/1.73 m²: Start with 10 mg; no dose adjustment needed 1
- eGFR 20-30 mL/min/1.73 m²: Can initiate for cardiovascular/renal benefits (not for glycemic control alone) 1
- eGFR <20 mL/min/1.73 m² or on dialysis: Contraindicated 1
Important caveat: Glucose-lowering efficacy diminishes significantly below eGFR 45 mL/min/1.73 m², but cardiovascular and renal protective benefits persist, so continuation is recommended until dialysis 3, 1
Pediatric Patients (Ages 10-17 years)
- Start with 10 mg once daily 1
- Can increase to 25 mg if HbA1c target not achieved after 12 weeks 1
- Higher hypoglycemia risk compared to adults, regardless of concomitant insulin use 1
Elderly Patients (≥75 years)
- Start with standard 10 mg dose 1
- Monitor closely for volume depletion and urinary tract infections, which occur more frequently in this age group 1
Clinical Context
The 10 mg starting dose balances efficacy with safety. While 25 mg shows slightly superior glycemic control in network meta-analyses 2, starting at 10 mg allows assessment of tolerability, particularly regarding:
- Genital mycotic infections (more common in women)
- Volume depletion risk
- Urinary tract infections
The current ADA guidelines 3 position SGLT2 inhibitors like empagliflozin as foundational therapy for patients with established cardiovascular disease, heart failure, or chronic kidney disease, often independent of baseline HbA1c or metformin use—emphasizing their cardiorenal protective effects beyond glucose lowering.
Common Pitfalls to Avoid
- Don't withhold in moderate renal impairment: Continue empagliflozin for cardiovascular/renal benefits even when eGFR falls below 45 mL/min/1.73 m² 3, 1
- Don't forget sick-day planning: Discontinue 3-4 days before scheduled surgery or during critical illness to mitigate rare DKA risk 3
- Monitor volume status: Particularly in elderly patients, those on diuretics, or with baseline hypotension 1
