Jangan Intubasi Dulu - Coba NIV Dulu!

Pada gagal nafas tipe 2 (hiperkapnik), pasien HARUS dicoba dengan NIV (ventilasi non-invasif/tiup-tiup balon) terlebih dahulu, KECUALI ada indikasi intubasi segera seperti henti nafas, pH <7.15 setelah resusitasi awal, atau penurunan kesadaran berat (GCS <8). 1

Kapan Mulai NIV

NIV harus dimulai ketika 1:

  • pH <7.35 DAN PaCO2 >6.5 kPa (≈49 mmHg) meskipun sudah dapat terapi medis optimal
  • Target saturasi oksigen: 88-92% (bukan 100%!)

Bahkan pada asidosis berat (pH <7.25), NIV masih boleh dicoba ASALKAN 1:

  • Tim terlatih tersedia
  • Akses cepat ke intubasi jika gagal
  • Monitoring ketat

Indikasi Intubasi Langsung (JANGAN Coba NIV)

Intubasi segera diperlukan jika 1:

  • Henti nafas atau hampir henti nafas
  • Distres respirasi sangat berat
  • pH <7.15 (setelah resusitasi awal dan oksigen terkontrol)
  • Penurunan kesadaran (GCS <8)
  • Syok kardiogenik
  • Kontraindikasi NIV: muntah berulang, obstruksi jalan nafas atas, deformitas wajah berat

Kapan NIV Dianggap Gagal

NIV gagal jika dalam 1-4 jam pertama 2, 3:

  • pH tetap turun atau tidak membaik
  • Respiratory rate tidak turun
  • Pasien makin distres

PENTING: Jangan terus memaksakan NIV jika gagal - ini meningkatkan risiko kematian karena keterlambatan intubasi 1. Mayoritas pasien yang perlu intubasi (78%) akan jelas dalam 8 jam pertama, dan 93% dalam 24 jam 3.

Algoritma Praktis

Langkah 1 - Penilaian Awal:

  • Cek AGD: pH dan PaCO2
  • Nilai kesadaran (GCS)
  • Nilai distres respirasi

Langkah 2 - Keputusan:

Jika ada salah satu:

  • Henti nafas/hampir henti
  • pH <7.15
  • GCS <8
  • Syok → INTUBASI LANGSUNG

Jika:

  • pH 7.15-7.35
  • Sadar/kooperatif
  • Tidak ada kontraindikasi → COBA NIV

Langkah 3 - Monitoring NIV (1-4 jam pertama):

  • Cek AGD ulang setelah 1 jam
  • Nilai pH, RR, distres

Jika membaik: Lanjutkan NIV Jika memburuk/stagnan: INTUBASI SEKARANG

Peringatan Penting

  • Jangan tunda intubasi jika NIV tidak efektif - ini fatal 1
  • Asidosis berat saja (pH <7.25) BUKAN kontraindikasi absolut NIV, tapi butuh tim sangat terlatih 1
  • Mayoritas kegagalan NIV terjadi dalam 8 jam pertama 3
  • Unit yang berpengalaman dengan protokol NIV yang baik mencapai tingkat keberhasilan 85% (hanya 15% perlu intubasi) 4

Catatan Khusus

Pada PPOK eksaserbasi akut, NIV terbukti menurunkan 2:

  • Kebutuhan intubasi
  • Lama rawat ICU
  • Mortalitas
  • Komplikasi infeksi

Namun manfaat ini HANYA jika NIV dimulai tepat waktu dan tidak ditunda ketika sudah jelas gagal.
