When and How to Start Dapagliflozin (SGLT2 Inhibitor)
Start dapagliflozin 10 mg once daily in adults with type 2 diabetes and eGFR ≥20 mL/min/1.73 m², heart failure with reduced ejection fraction (HFrEF) regardless of diabetes status and eGFR ≥20 mL/min/1.73 m², or chronic kidney disease with eGFR ≥25 mL/min/1.73 m² (especially with albuminuria ≥200 mg/g or heart failure). 1, 2
Patient Selection by Indication
Type 2 Diabetes with CKD
- Initiate at eGFR ≥20 mL/min/1.73 m² for cardiovascular and kidney protection 1, 2
- Highest priority: patients with albuminuria ≥200 mg/g (≥20 mg/mmol) 3
- For glycemic control alone: only use if eGFR ≥45 mL/min/1.73 m² (glucose-lowering effect diminishes below this threshold) 4, 1
Heart Failure with Reduced Ejection Fraction
- Start at eGFR ≥20 mL/min/1.73 m² regardless of diabetes status 5, 6
- Can initiate during hospitalization for acute HF once clinically stable and tolerating oral intake 3, 7
- NYHA class II-IV with elevated natriuretic peptides 6
Chronic Kidney Disease
- Initiate at eGFR ≥25 mL/min/1.73 m² 4, 1
- Strong recommendation for eGFR 20-45 mL/min/1.73 m² with albuminuria ≥200 mg/g 2
- Weaker recommendation for eGFR 20-45 mL/min/1.73 m² with albuminuria <200 mg/g 2
- Do not initiate in polycystic kidney disease or patients requiring immunosuppression for kidney disease 8
Dosing Algorithm
Standard Dosing
- Type 2 diabetes (glycemic control): Start 5 mg daily, may increase to 10 mg if additional glucose lowering needed and eGFR ≥45 mL/min/1.73 m² 8
- HF, CKD, or CV risk reduction: Start 10 mg daily 4, 8
Renal Dose Adjustments
- eGFR ≥45 mL/min/1.73 m²: No adjustment needed
- eGFR 25-44 mL/min/1.73 m²: Use 10 mg daily for HF/CKD indications; not recommended for glycemic control 4, 1
- eGFR 20-24 mL/min/1.73 m²: May initiate 10 mg for HF indication; initiation not recommended for CKD (but may continue if already on therapy) 1, 8
- eGFR <20 mL/min/1.73 m²: Do not initiate; may continue if tolerated until dialysis 1, 8
Pre-Initiation Assessment
Required Testing
- Check eGFR and assess volume status 8
- Correct volume depletion before starting 1, 8
- Assess hypoglycemia risk (insulin or sulfonylurea use, HbA1c at/below goal) 3
High-Risk Features Requiring Caution
- Volume depletion risk: Concurrent diuretic use, tenuous volume status, history of AKI 3
- Hypoglycemia risk: Consider reducing insulin or sulfonylurea dose by 10-20% at initiation 3
- Genital infection history: Counsel on hygiene measures 1, 8
- Foot ulcers or immunosuppression: Relative contraindications 3
Safety Precautions and Monitoring
Contraindications
- History of serious hypersensitivity to dapagliflozin 8
- Type 1 diabetes (not indicated; increased ketoacidosis risk) 5, 8
- Dialysis 1, 8
Perioperative Management
Withhold dapagliflozin at least 3 days before major surgery or procedures with prolonged fasting to prevent ketoacidosis 8. Resume when clinically stable and tolerating oral intake 8.
Sick Day Protocol
Temporarily withhold during acute illness, excessive exercise, or alcohol intake 3. Maintain hydration, monitor glucose and ketones more frequently, seek medical attention early 3.
Expected eGFR Changes
An acute, reversible decline in eGFR of 3-5 mL/min/1.73 m² typically occurs within 2-4 weeks of initiation 1, 3. This is not a reason to discontinue therapy unless eGFR drop exceeds 30% 2. Continue therapy even if eGFR falls below initiation threshold 1, 3.
Follow-Up Monitoring
- Reassess volume status, blood pressure, and renal function within 2-4 weeks 2
- Monitor for genital mycotic infections (6% incidence vs 1% placebo) 1, 8
- Screen for hypoglycemia if on insulin/sulfonylureas 3
- Watch for rare but serious: Fournier's gangrene (perineal necrotizing fasciitis), euglycemic ketoacidosis 1, 8
Common Pitfalls to Avoid
- Do not discontinue for mild eGFR decline: The initial dip is hemodynamic and protective long-term 1, 3
- Do not wait for "optimal" GDMT: Start early alongside other therapies; benefits appear within 12-28 days 6, 9
- Do not reduce diuretics preemptively: Only adjust if patient develops symptomatic hypotension or volume depletion 1, 3
- Do not use for glycemic control alone if eGFR <45 mL/min/1.73 m²: Glucose-lowering effect is minimal, but cardiovascular/renal benefits persist 4, 1
- Do not stop during hospitalization for HF: Can safely initiate or continue during acute decompensation once stabilized 3, 7
