Telephone Triage for Fatigue in Patient on Keytruda and Sacituzumab Govitecan
Use a numerical rating scale (0-10) immediately to quantify fatigue severity, then systematically screen for red flags including fever, severe diarrhea, dyspnea, confusion, or inability to perform basic self-care activities—patients scoring ≥4 or with any red flags require same-day evaluation. 1
Initial Severity Assessment
Start by asking the patient to rate their fatigue on a 0-10 scale:
- 0-3 = Mild fatigue
- 4-6 = Moderate fatigue
- 7-10 = Severe fatigue
This numerical rating scale is the validated screening tool recommended by ESMO guidelines for cancer-related fatigue 1. Fatigue is extremely common with this combination—occurring in 25% of patients on pembrolizumab alone 2 and up to 71% when combined with other agents 1.
Critical Red Flag Screen (Requires Urgent Evaluation)
Ask specifically about these danger signs that could indicate life-threatening complications:
Immune-Related Adverse Events from Pembrolizumab:
- Fever or chills (possible infection, adrenal crisis, or immune-mediated inflammation)
- Severe or bloody diarrhea (immune-mediated colitis—common with checkpoint inhibitors) 3
- Shortness of breath or new cough (pneumonitis—occurred in 1.8% leading to drug discontinuation) 2
- Confusion, severe headache, or vision changes (possible CNS involvement or endocrinopathy)
- Severe weakness preventing standing/walking (possible adrenal insufficiency—pembrolizumab can cause isolated ACTH deficiency) 4
Sacituzumab Govitecan-Specific Toxicities:
- Severe diarrhea (most common significant toxicity with sacituzumab) 5, 6
- Fever with neutropenia risk (neutropenia is the most common grade 3-4 adverse event) 5, 6
- Severe nausea/vomiting (very common with this agent)
- Bleeding or easy bruising (thrombocytopenia or anemia)
Triage Algorithm
If Fatigue Score 7-10 OR Any Red Flags Present:
- Direct to emergency department or same-day clinic evaluation
- High suspicion for:
If Fatigue Score 4-6 (Moderate) Without Red Flags:
- Schedule clinic visit within 24-48 hours for comprehensive assessment
- Patient needs:
- Complete blood count (anemia, neutropenia, thrombocytopenia)
- Comprehensive metabolic panel (electrolytes, renal function, liver function)
- Thyroid function tests (hypothyroidism in 15% on pembrolizumab) 2
- Morning cortisol level (screen for adrenal insufficiency) 4
- Assessment of contributing factors: pain, depression, sleep disturbances, nutritional status 1
If Fatigue Score 1-3 (Mild) Without Red Flags:
- Provide supportive care advice and schedule routine follow-up
- Counsel on:
- Expected symptom (fatigue occurs in majority of patients on this combination)
- Importance of maintaining light physical activity despite fatigue 1
- Adequate hydration and nutrition
- Sleep hygiene
- Schedule follow-up within 1-2 weeks or at next planned visit
- Instruct to call back immediately if fatigue worsens or red flags develop
Key Clinical Pitfalls to Avoid
Do not dismiss fatigue as "just expected"—while fatigue is common with both agents, severe fatigue can herald life-threatening complications. Recent evidence shows baseline fatigue predicts higher risk of severe and even fatal treatment toxicities 7, making this symptom a critical early warning sign.
Do not miss adrenal insufficiency—pembrolizumab can cause isolated ACTH deficiency presenting primarily as fatigue without the classic hyperpigmentation or hypotension 4. This is potentially fatal if untreated but rapidly reversible with corticosteroids.
Do not overlook infection risk—neutropenia from sacituzumab govitecan may not present with fever initially, and fatigue may be the predominant early symptom 5, 6.
Documentation and Follow-up
Document the fatigue severity score, presence/absence of red flags, and disposition. Ensure the patient understands when to seek immediate care versus routine follow-up. The combination of pembrolizumab and sacituzumab govitecan creates overlapping toxicity profiles requiring heightened vigilance 8, 9.
