Recommended Chemotherapy Regimens for Breast Cancer

HER2-Positive Disease

For HER2-positive breast cancer, the preferred first-line regimen is trastuzumab + pertuzumab + taxane (docetaxel or paclitaxel), with trastuzumab continued for 1 year total duration. 1, 2

First-Line Treatment (Adjuvant/Neoadjuvant)

Preferred regimens:

  • AC → Paclitaxel + Trastuzumab ± Pertuzumab 3, 4

    • Doxorubicin 60 mg/m² IV + Cyclophosphamide 600 mg/m² IV every 21 days × 4 cycles
    • Followed by Paclitaxel 80 mg/m² IV weekly × 12 weeks
    • With Trastuzumab 4 mg/kg loading, then 2 mg/kg weekly (or 8 mg/kg loading, then 6 mg/kg every 3 weeks)
    • Continue trastuzumab to complete 1 year total
    • Pertuzumab can be added for T2 or N1 disease 3
  • TCH ± Pertuzumab (anthracycline-free option) 3, 4

    • Docetaxel 75 mg/m² + Carboplatin AUC 6 + Trastuzumab every 21 days × 6 cycles
    • Preferred for patients with cardiac concerns due to lower cardiotoxicity 4

Critical sequencing: Trastuzumab should be given concurrently with taxane, NOT with anthracycline, due to significant cardiac toxicity risk 5, 4. Cardiac monitoring required at baseline, 3,6, and 9 months 5.

Metastatic/Advanced HER2-Positive Disease

First-line: Trastuzumab + Pertuzumab + Taxane 1, 2

  • Pertuzumab 840 mg IV loading, then 420 mg every 21 days
  • Trastuzumab 8 mg/kg loading, then 6 mg/kg every 21 days
  • Docetaxel 75-100 mg/m² every 21 days OR Paclitaxel 80 mg/m² weekly 6

Second-line: Trastuzumab emtansine (T-DM1) 3.6 mg/kg IV every 21 days if progression on first-line 1, 2

Updated second-line (2022): Trastuzumab deruxtecan (T-Dxd) is now preferred over T-DM1 as second-line treatment 2

Third-line and beyond: 1

  • Lapatinib 1250 mg PO daily + Capecitabine 1000 mg/m² PO twice daily days 1-14, every 21 days 5
  • Trastuzumab + other chemotherapy agents
  • Trastuzumab + Lapatinib (1000 mg PO daily when combined with trastuzumab) 5

Timing consideration: If trastuzumab-based adjuvant treatment finished >12 months before recurrence, follow first-line recommendations; if ≤12 months, follow second-line recommendations 1.

HER2-Negative Disease

Preferred Adjuvant Regimens 3, 4, 7

  • Dose-dense AC → Paclitaxel every 2 weeks (with filgrastim support)

    • Doxorubicin 60 mg/m² + Cyclophosphamide 600 mg/m² every 14 days × 4 cycles
    • Followed by Paclitaxel 175 mg/m² every 14 days × 4 cycles
  • Dose-dense AC → Weekly Paclitaxel

    • AC as above, followed by Paclitaxel 80 mg/m² weekly × 12 weeks
  • TC (Docetaxel + Cyclophosphamide)

    • Docetaxel 75 mg/m² + Cyclophosphamide 600 mg/m² every 21 days × 4 cycles
    • Alternative to AC regimens with improved DFS and OS, no anthracycline exposure 4, 7

Other Acceptable Regimens 3, 5

  • TAC: Docetaxel 75 mg/m² + Doxorubicin 50 mg/m² + Cyclophosphamide 500 mg/m² every 21 days × 6 cycles (all with filgrastim)
  • FEC → Docetaxel: Fluorouracil 500 mg/m² + Epirubicin 100 mg/m² + Cyclophosphamide 500 mg/m² × 3 cycles → Docetaxel × 3 cycles
  • CMF: Cyclophosphamide (oral days 1-14) + Methotrexate + Fluorouracil (IV days 1 and 8) every 28 days × 6 cycles 4

Triple-Negative Breast Cancer (TNBC) Specific Options 7

  • High-risk TNBC: Pembrolizumab + Carboplatin + Paclitaxel (neoadjuvant), followed by pembrolizumab + AC, then adjuvant pembrolizumab
  • TNBC with residual disease after neoadjuvant therapy: Capecitabine 1000-1250 mg/m² PO twice daily days 1-14, every 21 days 7
  • TNBC with germline BRCA1/2 mutations: Add olaparib 300 mg PO twice daily × 1 year if ≥pT2 or ≥pN1 after adjuvant chemotherapy, or residual disease after neoadjuvant therapy 7

Metastatic HER2-Negative Disease 5

Single agents:

  • Paclitaxel 175 mg/m² every 21 days OR 80-90 mg/m² weekly
  • Docetaxel 80-100 mg/m² every 21 days OR 35 mg/m² weekly
  • Capecitabine 1000-1250 mg/m² PO twice daily days 1-14, every 21 days
  • Gemcitabine 800-1200 mg/m² IV days 1,8,15 every 28 days
  • Vinorelbine 25 mg/m² IV weekly
  • Eribulin 1.4 mg/m² IV days 1 and 8, every 21 days
  • Albumin-bound paclitaxel 100 mg/m² days 1,8,15 every 28 days OR 260 mg/m² every 21 days

Hormone Receptor-Positive Disease

Chemotherapy and endocrine therapy should be given sequentially, with endocrine therapy AFTER chemotherapy completion 5. Use same chemotherapy regimens as HER2-negative disease above.

For HR+/HER2+ metastatic disease: May consider endocrine therapy + trastuzumab or lapatinib in selected cases (low disease burden, long disease-free interval, contraindications to chemotherapy), though HER2-targeted therapy + chemotherapy remains preferred for most patients 1.

Dose Modifications

Neutropenia

  • Grade 3-4 neutropenia: Hold chemotherapy until ANC >1000-1500/μL, then resume with dose reduction or add/continue G-CSF support
  • Febrile neutropenia: Hold until resolution, resume with 20-25% dose reduction and mandatory G-CSF
  • Dose-dense regimens require filgrastim 5 mcg/kg support 5, 7

Thrombocytopenia

  • Platelets <75,000/μL: Hold chemotherapy
  • Platelets 75,000-100,000/μL: Consider 25% dose reduction
  • Resume when platelets >100,000/μL

Neuropathy

  • Grade 2 (moderate symptoms): Reduce taxane dose by 20%
  • Grade 3 (severe symptoms): Hold taxane until improvement to ≤Grade 1, then resume at 50% dose or switch to different agent
  • Grade 4: Discontinue taxane permanently

Cardiac Toxicity (HER2-targeted therapy)

  • LVEF decline to 40-49% or 10-15% absolute decrease from baseline: Hold trastuzumab/pertuzumab, repeat LVEF in 3 weeks
  • LVEF <40%: Discontinue HER2-targeted therapy 5

Critical Considerations

Anthracycline cumulative dose limits: Doxorubicin ≤240 mg/m² (two-drug regimens) or ≤360 mg/m² (three-drug regimens); Epirubicin ≤600-720 mg/m² 4

Radiation timing: All chemotherapy (except CMF) should be completed before radiotherapy. CMF may be given concurrently with radiation 5.

Chemotherapy duration in metastatic setting: Continue for 4-6 months or to maximal response, then continue HER2-targeted therapy alone (if applicable) until progression 1.
