Polyp Surveillance Colonoscopy Schedule
For 1-2 small tubular adenomas (<10 mm), perform surveillance colonoscopy at 7-10 years; for 3-4 small adenomas, at 3-5 years; and for high-risk features (≥10 mm size, high-grade dysplasia, tubulovillous/villous histology, or 5-10 adenomas), at 3 years. 1
Risk Stratification Algorithm
Low-Risk Adenomas (7-10 year interval)
- 1-2 tubular adenomas <10 mm with low-grade dysplasia
- This represents a key update from previous 5-10 year recommendations, now extended to 7-10 years based on CRC outcome data rather than just advanced adenoma risk 1
- Recent validation shows this extended interval does not significantly increase neoplastic polyp detection compared to earlier surveillance 2
Intermediate-Risk Adenomas (3-5 year interval)
- 3-4 tubular adenomas <10 mm
- The 3-5 year range provides flexibility based on quality of baseline examination and patient factors 1
High-Risk Adenomas (3 year interval)
Surveillance at 3 years is indicated for any of the following 1:
- Adenoma ≥10 mm in size
- Tubulovillous or villous histology
- High-grade dysplasia
- 5-10 adenomas (regardless of size if <10 mm)
Very High-Risk (1 year interval)
- >10 adenomas warrants 1-year follow-up and genetic counseling for possible polyposis syndrome 1
Serrated Polyp Surveillance
Small serrated polyps (<10 mm without dysplasia) follow low-risk protocols; large serrated polyps (≥10 mm) or those with any dysplasia follow high-risk protocols 1, 3
- Serrated lesions have higher incomplete resection rates (RR 3.7) and are overrepresented in interval cancer series 4
- Consider 2-6 month site check for non-pedunculated serrated polyps 10-19 mm or those with dysplasia if complete excision uncertain 4
Critical Quality Requirements
These surveillance intervals apply only after high-quality baseline colonoscopy with 1, 4:
- Complete polyp removal documented
- Adequate bowel preparation
- Appropriate withdrawal time
- All detected neoplastic lesions removed
Important Caveats
Incomplete excision risk: Non-pedunculated polyps 10-20 mm have 18.3% incomplete resection rates versus 3.5% for pedunculated polyps 4. When histologic completeness cannot be determined for non-pedunculated polyps 10-19 mm, high-grade dysplasia, or dysplastic serrated polyps, perform site check at 2-6 months before determining surveillance schedule 4.
Subsequent surveillance adjustments: If high-risk adenomas are found at first surveillance, continue 3-year intervals. If no high-risk adenomas at first surveillance in initially high-risk patients, extend to 5-year intervals 3.
British guidelines divergence: UK guidelines require at least 2 premalignant polyps to qualify for surveillance (except post-CRC patients) and do not include tubulovillous/villous histology due to poor inter-observer agreement among pathologists 4. However, US guidelines 1 remain the most comprehensive and evidence-based for CRC outcomes.
Polyp Measurement Standards
Use histopathologic measurement as the largest dimension of neoplastic tissue. For piecemeal resection or fragmented specimens, use endoscopic size assessment 4. This addresses significant inter-observer variation in visual size estimation, which often underestimates true polyp size.
