Contraindications to Clonidine
The only absolute contraindication to clonidine is known hypersensitivity to the drug itself. 1
Absolute Contraindication
- Hypersensitivity to clonidine - This includes patients who have developed allergic reactions such as generalized rash, urticaria, or angioedema to clonidine in any formulation (oral or transdermal patch) 1
Critical Precautions (Relative Contraindications)
While not absolute contraindications, the following conditions require extreme caution or may preclude clonidine use:
Cardiac Conduction Abnormalities
- Sinus node dysfunction or AV block - Clonidine's sympatholytic action can worsen these conditions, particularly when combined with other sympatholytic drugs (beta-blockers, digoxin, calcium channel blockers) 1
- Post-marketing reports document severe bradycardia requiring IV atropine, isoproterenol, and temporary pacing in patients with conduction abnormalities taking clonidine 1
- Recent evidence confirms significantly increased risk of bradycardia (OR: 1.653) and hypotension (OR: 3.281) with perioperative clonidine use 2
Pheochromocytoma
- No therapeutic effect expected in hypertension caused by pheochromocytoma 1
Prior Allergic Reactions to Transdermal Clonidine
- Patients with localized contact sensitization to clonidine patches may develop generalized skin reactions if switched to oral clonidine 1
Important Clinical Caveats
Abrupt Discontinuation
While not a contraindication to starting clonidine, abrupt cessation is absolutely contraindicated once therapy begins. The 2017 ACC/AHA guidelines emphasize that clonidine must be tapered to avoid rebound hypertension and potential hypertensive crisis 3. This makes clonidine generally reserved as a last-line agent, particularly in older adults due to significant CNS adverse effects 3.
Drug Interactions Requiring Monitoring
- Concomitant use with agents affecting sinus node function or AV conduction (digitalis, calcium channel blockers, beta-blockers) requires careful heart rate monitoring 1
- Tricyclic antidepressants may reduce clonidine's hypotensive effect 1
Perioperative Considerations
Clonidine should be continued until 4 hours before surgery and resumed as soon as possible postoperatively, with careful blood pressure monitoring 1
Bottom line: Clonidine has remarkably few absolute contraindications—essentially only drug hypersensitivity. However, significant cardiac conduction disease represents a strong relative contraindication due to risk of severe bradycardia requiring intervention.
