Paxil (Paroxetine) Dosing
Start paroxetine at 20 mg once daily in the morning for major depressive disorder, 12.5 mg/day for panic disorder and social anxiety disorder, and reduce the starting dose to 12.5 mg/day (maximum 50 mg/day) in elderly patients, those with hepatic impairment, or severe renal impairment. 1
Standard Adult Dosing by Indication
Major Depressive Disorder
- Initial dose: 25 mg/day (Paxil CR) or 20 mg/day (immediate-release)
- Therapeutic range: 25-62.5 mg/day (CR formulation)
- Titration: Increase by 12.5 mg/day increments at weekly intervals if inadequate response
- Maximum dose: 62.5 mg/day (CR) or 50 mg/day (immediate-release) 1
Panic Disorder
- Initial dose: 12.5 mg/day (CR formulation)
- Therapeutic range: 12.5-75 mg/day
- Titration: Increase by 12.5 mg/day at weekly intervals
- Maximum dose: 75 mg/day 1
Social Anxiety Disorder
- Initial dose: 12.5 mg/day
- Therapeutic range: 12.5-37.5 mg/day (most patients respond to 20-50 mg/day)
- Titration: Increase by 12.5 mg/day at weekly intervals
- Maximum dose: 37.5 mg/day (CR) 1, 2
Obsessive-Compulsive Disorder
- Initial dose: 20 mg/day
- Effective doses: 40-60 mg/day (20 mg/day is often insufficient)
- Therapeutic range: 40-60 mg/day for optimal response
- Maximum dose: 60 mg/day 3, 4
Critical caveat: For OCD specifically, higher doses (40-60 mg/day) are significantly more effective than 20 mg/day, which showed no superiority over placebo in controlled trials 4. Do not settle for 20 mg/day in OCD patients.
Special Populations
Elderly Patients
- Initial dose: 12.5 mg/day
- Therapeutic range: 20-40 mg/day
- Maximum dose: 50 mg/day
- Pharmacokinetics are altered with increased plasma concentrations and prolonged half-life 1, 5
Hepatic or Severe Renal Impairment
- Initial dose: 12.5 mg/day
- Maximum dose: 50 mg/day
- Dose escalation should be more conservative due to reduced clearance 1
Pediatric Patients
Paroxetine is NOT indicated for use in children or adolescents due to increased risk of treatment-emergent suicidality 3. Fluoxetine is the only SSRI FDA-approved for pediatric depression.
Administration Guidelines
- Timing: Single daily dose, usually in the morning, with or without food 1
- Paxil CR tablets: Must be swallowed whole; do NOT chew or crush 1
- Time to response: Assess therapeutic response at 6-8 weeks; some patients may show improvement within 1-3 weeks 6, 5
Maintenance Therapy Duration
- First episode depression: Continue for 4-9 months after satisfactory response 6
- Recurrent depression (≥2 episodes): Consider longer-term maintenance therapy 6
- Panic disorder: Long-term maintenance is reasonable given the chronic nature of the condition 1
- Social anxiety disorder: Continue treatment for responding patients; this is a chronic condition 1
Dose Adjustment Strategy
If inadequate response after 6-8 weeks:
- Verify medication adherence
- Increase dose by 10-12.5 mg increments
- Wait at least 1 week between dose increases
- Do not exceed maximum recommended doses 6, 1
Discontinuation Protocol
Never stop paroxetine abruptly. Paroxetine has the highest risk of discontinuation syndrome among SSRIs 7, 8.
- Taper gradually rather than stopping abruptly
- If intolerable symptoms occur during taper, resume previous dose and decrease more slowly
- Monitor for discontinuation symptoms: dizziness, sensory disturbances, anxiety, insomnia 1
Drug Interactions and Contraindications
- MAOIs: Allow 14 days between discontinuing an MAOI and starting paroxetine, and vice versa 1
- Linezolid/methylene blue: Do not initiate paroxetine in patients receiving these agents due to serotonin syndrome risk 1
- CYP2D6 considerations: Paroxetine is metabolized by CYP2D6; poor metabolizers may require lower doses 3
Pregnancy Considerations
- Category D in pregnancy: Particularly concerning in third trimester due to neonatal complications
- Consider tapering in third trimester after careful risk-benefit assessment 1
- Recent data suggest increased risk of congenital malformations, though evidence is not conclusive 7
Key Clinical Pearls
The evidence strongly supports that paroxetine requires adequate dosing for efficacy—particularly in OCD where 40-60 mg/day is necessary 4. Starting too low or failing to titrate appropriately is a common pitfall. However, paroxetine also carries higher risks of sedation, constipation, sexual dysfunction, weight gain, and discontinuation syndrome compared to other SSRIs 7, 8, making careful patient selection and monitoring essential.
