Nebivolol Starting Dose
For adults with hypertension, start nebivolol at 5 mg once daily, which can be titrated up to 40 mg daily if needed; however, reduce the starting dose to 2.5 mg once daily in patients ≥65 years with severe renal impairment (CrCl <30 mL/min), moderate hepatic impairment, or those taking strong CYP2D6 inhibitors, and for heart failure with reduced ejection fraction, start at 1.25 mg once daily. 1
Hypertension Dosing
The FDA-approved starting dose for hypertension is 5 mg once daily, taken with or without food 1. This applies to most adult patients and can be used as monotherapy or combined with other antihypertensive agents 2, 3. The 2017 ACC/AHA hypertension guidelines list nebivolol's usual dose range as 5-40 mg once daily 2, 3.
Titration strategy: If blood pressure remains uncontrolled, increase the dose at 2-week intervals up to a maximum of 40 mg daily 1. More frequent dose adjustments are unlikely to provide additional benefit 1.
Special Populations Requiring Dose Reduction
Severe renal impairment (CrCl <30 mL/min): Start at 2.5 mg once daily and titrate slowly as needed 1. Nebivolol has not been studied in dialysis patients 1.
Moderate hepatic impairment: Start at 2.5 mg once daily and titrate slowly 1. Avoid nebivolol entirely in severe hepatic impairment, as it has not been studied in this population 1.
Strong CYP2D6 inhibitors: While the FDA label states no formal dose adjustment is required for CYP2D6 poor metabolizers 1, clinical judgment suggests starting at 2.5 mg daily when combining with strong CYP2D6 inhibitors (e.g., fluoxetine, paroxetine, quinidine) to avoid excessive beta-blockade.
Elderly patients (≥65 years): No routine dose adjustment is necessary based solely on age 1. However, if elderly patients have concurrent severe renal or moderate hepatic impairment, use the reduced 2.5 mg starting dose 1.
Heart Failure with Reduced Ejection Fraction (HFrEF)
For HFrEF, the starting dose is substantially lower: 1.25 mg once daily 4, 5. This recommendation comes from the 2016 ESC Heart Failure Guidelines, which specify this dose for patients with LVEF ≤40% 4.
Target dose for HFrEF: Titrate up to 10 mg once daily over several weeks 4, 5. The 2008 ESC guidelines recommend doubling the dose every 2-4 weeks if tolerated, avoiding increases if signs of worsening heart failure, symptomatic hypotension, or excessive bradycardia (pulse <50 bpm) occur 5.
Patient requirements for HFrEF initiation 5:
- LVEF ≤40%
- NYHA class II-IV symptoms
- Clinically stable (no recent diuretic changes)
- Already on optimal ACEI/ARB dose
- Not dependent on IV inotropes
Key Clinical Considerations
Contraindications 5:
- Asthma (COPD is NOT a contraindication)
- Second- or third-degree heart block without pacemaker
- Sick sinus syndrome without pacemaker
- Sinus bradycardia <50 bpm
Avoid abrupt discontinuation: Nebivolol should be tapered gradually to prevent rebound hypertension or cardiac events 2, 3, 6.
Monitoring: Watch for symptomatic hypotension, bradycardia, and worsening heart failure, particularly during titration 5. In HFrEF patients, if hypotension occurs, consider reducing other vasodilators before decreasing nebivolol 5.
Unique properties: Nebivolol differs from other beta-blockers through its nitric oxide-mediated vasodilation 2, 3, which may provide advantages in certain patients, though this doesn't alter the starting dose recommendations.
