Management of Type 2 Diabetes with Stage 3b CKD
Start an SGLT2 inhibitor immediately (if not already on one) and metformin, as this patient has type 2 diabetes (fasting glucose 155 mg/dL) with stage 3b CKD (eGFR 40 mL/min/1.73 m²), and both medications are indicated and safe at this level of kidney function. 1, 2
Immediate Medication Management
First-Line Therapy
Initiate SGLT2 inhibitor (dapagliflozin, empagliflozin, or canagliflozin) - this is critical as the eGFR of 40 mL/min/1.73 m² is well above the threshold of ≥20 mL/min/1.73 m² for initiation 1
- SGLT2 inhibitors provide kidney protection, slow GFR decline, and reduce cardiovascular events independent of glucose-lowering effects at this eGFR level
- The initial reversible eGFR dip after starting SGLT2i should not prompt discontinuation 1
Continue or initiate metformin at appropriate dose for eGFR 30-44 mL/min/1.73 m² 2
- With eGFR 40, metformin is safe but requires dose adjustment
- Consider reducing to 1000-1500 mg daily (maximum 2000 mg/day in this eGFR range)
- Monitor eGFR every 3-6 months given the level is <60 mL/min/1.73 m² 2
Additional Glucose Control if Needed
If glycemic targets are not met with metformin plus SGLT2i:
- Add GLP-1 receptor agonist (liraglutide, semaglutide, or dulaglutide) as the preferred third agent 1, 2
- These agents retain glucose-lowering potency at eGFR 40 and provide cardiovascular benefits
- They reduce ASCVD events and albuminuria
- Start at lowest dose and titrate slowly to minimize GI side effects (nausea occurs in 15-20% but usually resolves) 1
Assess the Elevated BUN/Creatinine Pattern
The BUN of 51 mg/dL with creatinine 2.23 mg/dL gives a ratio of 23 (slightly elevated from normal 9-20):
This mildly elevated ratio suggests a prerenal component - assess for:
- Volume depletion (especially important before starting SGLT2i)
- Dietary protein intake
- GI bleeding or increased protein catabolism
- Medication effects (NSAIDs, ACE inhibitors/ARBs)
However, the ratio is not dramatically elevated (would expect >30 for pure prerenal azotemia), suggesting the kidney dysfunction is primarily intrinsic CKD 3
Critical Monitoring Strategy
Before Starting SGLT2 Inhibitor
- Ensure adequate hydration status
- Consider reducing diuretic dose if patient is on high-dose diuretics to prevent volume depletion 1
- Educate on sick-day protocol: hold SGLT2i during acute illness
Ongoing Monitoring
- eGFR every 3-6 months given level <60 mL/min/1.73 m² 2
- Expect initial eGFR dip of 3-5 mL/min/1.73 m² after SGLT2i initiation - this is hemodynamic and protective, not harmful 1
- Continue SGLT2i even if eGFR falls below 40 during treatment (can continue down to eGFR 20) 1
- Monitor vitamin B12 annually on metformin 1
- Adjust insulin or sulfonylurea doses if used concurrently to prevent hypoglycemia 1
Glycemic Targets
- Set individualized HbA1c target (typically 7-8% for CKD stage 3b to balance glycemic control with hypoglycemia risk)
- Fasting glucose goal: 80-130 mg/dL
- Current fasting glucose of 155 mg/dL requires treatment intensification
Lifestyle Modifications
- Weight loss of 5-7% if overweight 2
- Moderate-intensity physical activity ≥150 minutes per week 2
- Adequate hydration (especially important with SGLT2i)
- Dietary sodium restriction and protein moderation for CKD
Common Pitfalls to Avoid
- Do not stop metformin at eGFR 40 - it's contraindicated only below 30 mL/min/1.73 m² 2
- Do not withhold SGLT2i due to the initial eGFR dip - this is expected and protective 1
- Do not delay SGLT2i initiation - kidney protection benefits are time-sensitive
- Do not use sulfonylureas as preferred agents - they increase hypoglycemia risk in CKD 1
