Duration of Alendronate/Cholecalciferol Treatment
Treat with alendronate 70 mg weekly and cholecalciferol 800-1000 IU daily for an initial period of 3-5 years, then reassess fracture risk to determine whether to continue therapy or take a bisphosphonate holiday.
Initial Treatment Period
The 2019 Endocrine Society guidelines provide clear direction: after 3 to 5 years of bisphosphonate therapy, fracture risk must be reassessed 1. This is not optional—it's a critical decision point that determines your next steps.
The Decision Point at 3-5 Years
At this reassessment, you have two paths:
Path 1: Continue Treatment
If you remain at high risk of fractures, continue alendronate therapy beyond 5 years 1. High-risk features include:
- T-score still ≤ -2.5 at the hip
- History of previous fractures, especially vertebral or hip
- Very advanced age (>75-80 years)
- Multiple risk factors for falls
- Glucocorticoid use
Path 2: Bisphosphonate Holiday
If you are at low-to-moderate fracture risk, consider a bisphosphonate holiday 1. This means:
- Temporary discontinuation for up to 5 years
- The holiday period may be longer depending on bone mineral density and clinical circumstances
- Evidence is strongest for alendronate (and zoledronic acid) showing retained benefits during the holiday 1
Critical Monitoring During a Holiday
Once you start a bisphosphonate holiday, reassess fracture risk every 2-4 years 1. You must restart therapy earlier than 5 years if:
- Significant decline in bone mineral density occurs
- Any new fracture occurs
- Clinical risk status changes (new glucocorticoid use, significant weight loss, etc.)
Why This Approach?
The rationale is based on bisphosphonates' unique property: their positive effects persist for several years after discontinuation 1. Randomized trials showed benefits continuing after stopping at 5 years for alendronate and 3 years for IV zoledronic acid 1.
However, the risk of atypical femoral fractures increases with longer duration—from ~3 per 100,000 person-years at 2 years to ~50 per 100,000 with >8 years of use 1. Despite this increase, the benefits of preventing typical osteoporotic fractures still outweigh risks, particularly in high-risk older women 1.
Monitoring During Treatment
Monitor bone mineral density by DXA at the spine and hip every 1-3 years to assess treatment response 1. Expected increases after 12 months of alendronate 70 mg weekly are approximately 7% at the lumbar spine and 5% at the total hip 1.
Cholecalciferol Duration
Continue cholecalciferol 800-1000 IU daily indefinitely, regardless of whether you continue or stop alendronate 1. Adequate vitamin D is fundamental for bone health and should be maintained throughout life. Recent evidence suggests calcifediol may be more effective than cholecalciferol at raising vitamin D levels, though both are safe 2, 3.
Common Pitfall to Avoid
Do not simply continue alendronate indefinitely without reassessment. The 3-5 year mark is mandatory for fracture risk re-evaluation. Failing to reassess exposes patients to unnecessary long-term bisphosphonate risks without confirming ongoing benefit.
